Excessive maternal weight and practice of exclusive breastfeeding among women of low socioeconomic status

ABSTRACT Objective: To investigate the impact of excessive maternal weight on the early discontinuation of exclusive breastfeeding Methods: This is a longitudinal study including mother-infant dyads of low socioeconomic status receiving prenatal care in Health Care Centers in Porto Alegre, Rio Grande do Sul, Brazil. A structured questionnaire was administered to women in the last trimester of pregnancy, including weight measurements. Another interview was conducted six months after delivery, and data on infant feeding practices were collected and maternal height was measured. Maternal nutritional status was assessed using body mass index values according to gestational age. Discontinuation of exclusive breastfeeding before 4 months was considered a low duration rate Results: A total of 619 mother-infant dyads were evaluated. The prevalence of maternal overweight in the third trimester of pregnancy was 51%. The median duration of exclusive breastfeeding was 2.0 months. After adjustment for possible confounding factors, no association between maternal overweight and early discontinuation of exclusive breastfeeding was identified. Maternal smoking was identified as a risk factor (1.23, 95%CI=1.13-1.35) for early discontinuation of exclusive breastfeeding Conclusion: Excessive maternal weight was not confirmed as a risk factor for early discontinuation of exclusive breastfeeding. However, women who reported being smokers had a higher risk of early discontinuation of exclusive breastfeeding than those who did not smoke. This indicates the need for public health interventions to promote smoking cessation during pregnancy and in the postpartum period because of the deleterious effects of this habit on maternal and infant health.

Dairy products consumption in Brazil is associated with socioeconomic and demographic factors: Results from the National Dietary Survey 2008-2009

ABSTRACT Objective: To estimate the consumption of dairy products in the Brazilian population. Methods: Data from two non-consecutive food records from 34,003 individuals (aged ³10 years) in the first Brazilian nationally representative dietary survey (2008-2009) were used to estimate the dairy products intake. Dairy products were divided into four major subgroups: cow's milk, cheese, yogurt and other dairy products. Absolute and relative frequencies of individuals in the population who reported consumption of dairy products in at least one of the two-day food record according to sex, age, geographic region and levels of per capita monthly family income were calculated. Moreover, the mean consumption of dairy products, as well as their nutritional contribution in total energy, protein, calcium, phosphorus and magnesium intake was estimated. Results: It was observed that only 43.0% of the population reported consuming dairy products, being the milk subgroup the most frequently consumed (21.1%). The prevalence of consumption was higher among females, elderly, residents of the South region, and among individuals from higher per capita monthly family income. The nutritional contribution of dairy products was less than 10.0% of total energy intake and for most nutrients and the mean daily consumption are justified was 101.8g. Conclusion: Therefore, in view of the low intake of dairy products by Brazilian population and the nutritional benefits associated with this food group, public health strategies to increase dairy consumption are justified.

Evaluation of a mobile application for estimation of food intake

ABSTRACT Objective: Evaluate the use of the Nutrabem (São Paulo, Brasil) mobile application as a tool for measurement of food intake among university students. Methods: Cross-sectional study of a random sample of 40 undergraduate students at the Universidade Federal de São Paulo, Campus Baixada Santista. Food intake data were estimated using the Nutrabem app and the 24-hour dietary recall. Intakes of energy, carbohydrates, proteins, lipids, calcium, iron, and vitamin C were calculated. The intake of food groups and diet quality were evaluated by the Diet Quality Index associated with the Digital Food Guide. The agreement between the methods was assessed using the Pearson's correlation coefficient and the Student' t-test. Results: Strong correlations were observed between energy (0.77), carbohydrates (0.82) and protein (0.83). The groups: poultry, fish, and eggs; beef and pork; refined grains and breads; and fruits and legumes showed strong correlations (between 0.76 and 0.85). There were moderate correlations (0.59 and 0.71) between the groups sugars and sweets; whole grains, tubers and roots, milk and dairy products, animal fats, and the Diet Quality Index associated with the Digital Food Guide scores. Vegetables and leafy greens, nuts, and vegetable oils showed weak correlations (0.31 and 0.43). Homogeneity assessment revealed similarity between the results obtained by both methods (p>0.05) . Conclusion: The Nutrabem app can be used as a tool to assess dietary intake among university students since it produces results similar to those obtained by the 24-hour dietary recall method.

Ketogenic food pyramid for patients with refractory epilepsy: From theory to clinical practice

ABSTRACT Objective: To develop a graphical representation in the form of a food pyramid for a ketogenic diet for dietary treatment in children and adolescents with refractory epilepsy. Methods: The pyramid was constructed based on: the estimation of energy requirements for different age groups, macronutrient distribution, food groups, and the number of servings and respective amounts of food according to the ketogenic diet. Serving sizes were based on the calculation of energy and macronutrient requirements according to age and nutritional status. Results: The pyramid was divided into three tiers and 5 food groups (fats, proteins, type 1 vegetables, type 2 vegetables, and fruits). Four portion size lists were defined for the following age groups: 1-3 years, 4-6 years, 7-10 years, and 11-19 years. Conclusion: The ketogenic diet food pyramid can be used as nutritional guidance for patients undergoing this dietary therapy by illustrating the variety of foods that can be eaten during the treatment, optimizing adherence to the treatment, and guaranteeing beneficial effects on seizure control.

Modeling of Food and Nutrition Surveillance in Primary Health Care

ABSTRACT Objective: To describe the modeling stages of food and nutrition surveillance in the Primary Health Care of the Unified Health Care System, considering its activities, objectives, and goals Methods: Document analysis and semi-structured interviews were used for identifying the components, describe the intervention, and identify potential assessment users. Results: The results include identification of the objectives and goals of the intervention, the required inputs, activities, and expected effects. The intervention was then modeled based on these data. The use of the theoretical logic model optimizes times, resources, definition of the indicators that require monitoring, and the aspects that require assessment, identifying more clearly the contribution of the intervention to the results Conclusion: Modeling enabled the description of food and nutrition surveillance based on its components and may guide the development of viable plans to monitor food and nutrition surveillance actions so that modeling can be established as a local intersectoral planning instrument.

Contamination by intestinal parasites in vegetables marketed in an area of Jequitinhonha Valley, Minas Gerais, Brazil

ABSTRACT Objective: The present study aimed to evaluate the presence of helminthes and intestinal protozoa in vegetables commercialized in Diamantina, a municipality located at Jequitinhonha Valley, one of the poorest regions of the world. Methods: A total of 108 specimens, including lettuce, green onion and rocket, were monthly collected from the most popular open street market, green grocery and supermarket of the municipality. The samples were processed by a concentration method and evaluated by light microscopy for parasitological identification. Results: The percentage of contamination was 50.9% (55/108), with predominance of nematode larvae (36.5%), cysts of Entamoeba coli (26.0%) and eggs of hookworms/Strongyloides spp. (12.9%). Lettuce showed greater contamination rate (61.1%) and samples from the open street market were more contaminated (77.8%). Information collected at each point of sale pointed the field cultivation as the critical step for such contaminations. Conclusion: Vegetables marketed in Diamantina presents a wide variety of intestinal parasites, which may represent a potential risk to the health of consumers of fresh vegetables.

The scientific production of Nutrition published by the Scientific Electronic Library under the gaze of the evaluation of the Coordination for the Development of Higher Education Personnel

ABSTRACT Objective: Perform analysis of the scientific production of Nutrition published by the Scientific Electronic Library under the gaze of the evaluation of Coordenação de Aperfeiçoamento de Pessoal de Nível Superior. Methods: Systematic search was performed in the 286 current title journals, using the "article search" path, the "subject index" window and the "Nutrition" indexer. The articles and periodicals selected were analyzed in order to answer the following questions: Which journals from Scientific Electronic Library publish articles on Nutrition? Do these journals have an impact factor as measured by the Journal Citation Reports? Which are? What is the classification of these journals by the areas of evaluation of Coordenação de Aperfeiçoamento de Pessoal de Nível Superior, specifically by the area of Nutrition? Results: We analyzed 779 articles published in 85 journals. The majority of articles (n=698; 89.6%) were related to the area of Health Sciences. The five journals with higher volumes of articles were: Brazilian Journal of Nutrition, Reports in Public Health, Public Health Journal, Science & Public Health and Journal of Pediatrics. Most (62.4%) does not have Journal Citation Reports. The Nutrition area not rated any journal in strata A1 and A2, classifying them from B1 to B4. For eight areas analyzed their specific journals were induced to the upper strata of Qualis (A1, A2 and B1). Conclusion: The analyzed journals are the main vehicles of circulation of scientific paradigms of national scientific communities. Considering the correlation of forces between the vehicles of circulation of scientific knowledge in the international context, stimuli for valorization and qualification of these journals are essential.

Use of dietary supplements among Brazilian athletes

ABSTRACT Objective: To investigate the prevalence and establish the profile of the consumption of dietary supplements among Brazilian athletes. Methods: A total of 182 athletes of both genders from 20 different sports participated in this study. The athletes answered a questionnaire containing sociodemographic and sports-related questions and were interviewed about the consumption of dietary supplements. Results: Forty seven percent of athletes reported having consumed at least one type of dietary supplement and 38% said they use more than three different types of supplements concurrently. Whey protein was the most commonly consumed supplement, and the most frequently mentioned reason for its consumption was performance improvement. The main source of information was coaches, and individual sport athletes were the greatest consumers and the most likely to seek dietary supplement. Conclusion: Approximately half of the participants used dietary supplements, which on most occasions were recommended by coaches. The consumption profile also revealed multiple supplementation practice and showed that nutrition education is essential for this specific population.

Association of insufficient physical activity with sarcopenia and sarcopenic obesity in individuals aged 50 years or more

ABSTRACT Objective: To analyze the association of insufficient physical activity in different domains with sarcopenia or sarcopenic obesity in patients aged ³50 years. Methods: The sample consisted of 770 males and females. Sarcopenia was diagnosed when the individual had: (1) low muscle mass and muscle strength; or (2) low walking speed and low muscle mass; sarcopenic obesity was diagnosed when individuals were at risk of obesity and sarcopenia. Muscle mass was given by a predictive equation, and then the muscle mass index (in kg/m²) was given by muscle mass divided by height squared. Muscle strength, in kg, was given by measuring handgrip strength with a digital dynamometer. The 4m walk test evaluated walking sped. Low muscle mass, muscle strength, and walking speed were defined as the respective values below the 25th percentile, and risk of obesity was defined as body mass index ³25kg/m2. Results: Habitual physical activity was investigated by a questionnaire. Insufficient leisure-time physical activity was associated with sarcopenia (OR=2.55; 95%CI=1.10-5.88). In addition, insufficient leisure-time physical activity (OR=4.75; 95%CI=1.64-13.72), leisure-time and commuting physical activities (OR=2.49; 95%CI=1.02-6.11, and habitual physical activity (OR=3.55; 95%CI=1.0-11.79) were associated with sarcopenic obesity. Conclusion: Insufficient physical activity is associated with sarcopenia or sarcopenic obesity in the study individuals aged ³50years.

Iron-deficiency anemia and associated factors among preschool children in Diamantina, Minas Gerais, Brazil

ABSTRACT Objective: Study the prevalence of iron depletion and iron-deficiency anemia and their associated factors in preschool children. Methods: Cross-sectional study with five-year old preschool children from a birth cohort of the city of Diamantina, Minas Gerais state, Brazil. Socioeconomic, demographic, and dietary characteristics were obtained through a questionnaire administered to each child mother or guardian. Iron depletion (normal hemoglobin and low serum ferritin levels) and iron-deficiency anemia (hemoglobin level than 11g/dL) were detected after collecting 5mL of venous blood of preschool children. Poisson regression was used to identify the factors associated with iron depletion and iron-deficiency anemia. Results: A total of 228 preschool were evaluated, corresponding to 97.4% of the children from a cohort study followed-up up to the end of their first year of life. Iron depletion and iron-deficiency anemia were detected, respectively, in 15.9% and 18.9% of the preschool children evaluated. Iron depletion was not associated with any variable studied, while low maternal education level was associated with iron-deficiency anemia (PR=1.83; P=0.03). Conclusion: Iron-deficiency anemia is considered as a mild public health problem among 5-year old children in the city of Diamantina, Minas Gerais. Higher maternal education level was a protective factor against this deficiency, and therefore it is as an important marker for the occurrence of iron-deficiency anemia in the population studied.

Dietary patterns of obese children: Maternal perceptions and experiences

ABSTRACT Objective: To understand maternal perceptions and experiences regarding the eating habits of obese children aged five to nine years. Methods: This is a qualitative research, and semi-structured interviews and discourse analysis were used to interpret narratives of 13 women from the city of Fortaleza, Ceará state, Brazil. Results: These women described the eating habits of their obese children in terms of how they eat and mentioned: eating fast, eating in front of the television, secret eating, eating large amounts of food, and the consumption of processed foods that are high in fat, sugars, and sodium. Conclusion: Seeing the mother and her obese child as a unit that needs support and guidance is a big step to plant the seeds to reap the rewards, i.e., exerting important impacts on the lives of these families and on the current scenario of childhood obesity.

Abdominal obesity and healthcare costs related to hypertension and diabetes in older adults

ABSTRACT Objective: To analyze the association between excess abdominal fat and healthcare costs related to hypertension and diabetes Mellitus in older adults. Methods: These cross-sectional analyses are part of the Health, Wellbeing and Aging Study conducted in São Paulo, Brazil, with 806 older adults with self-reported hypertension and diabetes Mellitus. The study included the annual costs with medicines, hospital admissions, and outpatient services for hypertension and diabetes Mellitus control. Excess abdominal fat was diagnosed based on waist circumference. Level of physical activity, age, and gender were considered covariates. The sample was divided into two groups according to waist circumference. Multiple logistic regression analyzed the associations between annual costs and waist circumference. Results: The cost of services and hospitalizations (R$551.05; 95%CI=418.27-683.83) and total costs (R$817.77; 95%CI=669.21-966.33) were higher in the excess abdominal fat group. Older adults with high waist circumference had higher odds of increasing annual costs due to medicines (OR=2.6; 95%CI=1.13-3.77), regardless of gender, age, and level of physical activity. Conclusion: Healthcare costs for treating hypertension and diabetes Mellitus in older adults are higher in the presence of excess abdominal fat.

Dietary patterns and significance of nutrition for women with low-risk pregnancy

ABSTRACT Objective: To evaluate dietary patterns and significance of diet for pregnant women. Methods: Cross-sectional study carried out in eight health units in Fortaleza, Ceará, Brazil, with 201 pregnant women. The following instruments were used: a socio-economic and health questionnaire, the Free-Word Association Test, and a Food Frequency Questionnaire. Dietary patterns were identified using principal components and factor analysis. Poisson regression with 5% significance level was used. Results: Three dietary patterns were identified: current Brazilian pattern (beans, rice, processed meats, fats, refined grains, pasta and pastries, soft drink, sugar and sweets, cookies and crackers); healthy pattern (fruits and fruit juices, vegetables, whole grains, seafood, dairy products); and energy-rich pattern (salty deep-fried snacks, popcorn, packaged snacks, instant noodles, tubers, and chicken). Women who did not receive nutrition guidance during prenatal care showed less chance of adherence to the current Brazilian dietary pattern (PR=0.87), and therefore their level of consumption of foods commonly present in Brazilian diets was low. For most women, the significance of diet was reported as important and healthy, but it was not associated with any of the diet patterns identified. However, the women who did not consider that during pregnancy diet should be healthy showed greater chance of adherence to the energy-rich pattern (PR=1.18). This finding deserves special attention since excessive weight gain can have a negative effect on pregnancy. Conclusion: Nutrition guidance during prenatal care and the way pregnant women perceive their eating habits can influence their food choices during pregnancy.

Agreement of dietary fiber and calorie intake values according to the choice of nutrient composition and household measure tables

ABSTRACT Objective: To analyze the variations in the daily intake of dietary fiber and calories according to the different nutrient composition and homemade measure tables. Methods: Five different methods based on different nutrient composition and household measure tables were used to calculate daily calorie and fiber intake, measured using a food frequency questionnaire, of 633 pregnant women receiving care in primary health care units in the Southern region of Brazil; they were selected to participate in a cohort study. The agreement between the five methods was evaluated using the Kappa and weighted Kappa coefficients. The Nutritional Support Table, a Brazilian traditional food composition table and the Brazilian household expenditure survey were used in Method 1. Brazilian Food Composition Table and the Table for the Assessment of Household Measures (Pinheiro) were used in Methods 2 and 3. The average values of all subtypes of food listed in the Brazilian Food Composition Table for each corresponding item in the food frequency questionnaire were calculated in the method 3. The United States Department of Agriculture Food Composition Table and the table complied by Pinheiro were used in Method 4. The Brazilian Food Composition Table and the Brazilian household expenditure survey were used in Method 5. Results: The highest agreement of calorie intake values were found between Methods 2 and 3 (Kappa=0.94; 0.92-0.95), and the lowest agreement was found between Methods 4 and 5 (Kappa=0.46; 0.42-0.50). As for the fiber intake, the highest agreement was found between Methods 2 and 5 (Kappa=0.87; 0.82-0.90), and the lowest agreement was observed between Methods 1 and 4 (Kappa=0.36; 0.3-0.43). Conclusion: Considerable differences were found between the nutritional composition tables. Therefore, the choice of the table can influence the comparability between studies.

Effect of home vegetable gardening on the household availability of fruits and vegetables

ABSTRACT Objective: Evaluate the effect of planting herb or vegetable seeds and seedlings on the household availability of fruits and vegetables through an intervention study combining various actions (educational practices, home visits, and distribution of seeds and seedlings) for the promotion of fruit and vegetable consumption in multiple settings. Methods: Data from 70 families were analyzed. Using the stratified sampling technique, the participant families were selected from a stratum within a population of 1,743 families living in three low-income communities in the city of Rio de Janeiro, Brazil. Three post-intervention groups were formed and compared: families that did not receive the seeds and seedlings during the intervention; families that received but did not plant the seeds and seedlings; families that received and planted the seeds and seedlings. Results: Among the families that did not grow their own fruits and vegetables before the intervention, those that received and planted the seeds and seedlings achieved an increase in household availability of fruits and vegetables (Δ=+5.8 percentage points) up to three times higher (p<0.05) than that achieved by the families who did not receive the seeds and seedlings (Δ=+1.8 5 percentage points) and those that received but did not plant them (Δ=+2.3 percentage points). As for the families that grew their own fruits and vegetables before the intervention, the comparison between those that did not receive the seeds and seedlings, those that received and planted them, and those that received but did not plant them showed no differences in the household availability of fruits and vegetables. Conclusion: The planting of the seeds and seedlings that they received by the families that did not grow their own fruits and vegetables before the intervention contributed to a significant increase in the household availability of fruits and vegetables.

Meanings attributed to healthy eating by consumers of a street market

ABSTRACT Objective: To investigate the meanings attributed to healthy eating by consumers of a street market in the region called Recôncavo da Bahia, Bahia, Brazil. Methods: Phenomenology-based ethnography to understand the meanings attributed by those consumers. Information was collected through participant observation documented in a field diary and in-depth interviews. Interviews were conducted with seven people who were visitors and/or worked at the street market. In the analytical process, the following significant concepts related to healthy eating were systematized: "fruits and vegetables represent healthy eating"; "safe food: it has to be clean", and "foods that do the body good". Results: The meanings attributed to healthy eating are revisited daily and are related to individuals' life experiences; new meanings derive from intersubjective constructions. Based on the interviews, it was observed that re-signification was a constant process, marked by events that influence the respondents to change their eating habits such as diseases, aging, information received by health care professionals, and media reports. Healthy eating was also represented by the hygienic-sanitary quality of foods and the lack of strict control over everyday food choices. Conclusion: We highlight the importance of this discussion in the scientific field and governmental bodies, as well as among health care professionals aiming at a better understanding of the different concepts of healthy eating.

Nutritional problems in older adults with Alzheimer’s disease: Risk of malnutrition and sarcopenia

ABSTRACT Objective Understand the nutritional problems and detect the presence of sarcopenia in older adults with Alzheimer’s disease. Methods Descriptive cross-sectional study carried out among elderly patients with Alzheimer’s disease receiving care at the Unidade de Saúde de Atenção ao Idoso (Elderly Care Unit) in a capital city in Southern Brazil between November 2010 and July 2011. The Clinical Dementia Rating scale was used for the evaluation of staging severity of dementia. Participants’ nutritional status was classified using The Mini Nutritional Assessment. The following tests were used to diagnose sarcopenia: bioelectrical impedance, hand grip strength, and the Timed Up and Go test. Anthropometric measurements and laboratory tests (hemoglobin, lymphocytes, serum albumin, and total cholesterol) were performed. Results Ninety-six older adults (mean age of 78 years) were evaluated. It was observed prevalence of mild Alzheimer’s disease in 54.2% of the participants; 55.2% were at risk of malnutrition; unintentional weight loss was observed in 64.6%, 55.3% had lower number of lymphocytes, and 43.7% had severe sarcopenia. Conclusion The prevalence of risk of malnutrition and sarcopenia is high among older adults with Alzheimer’s disease. Future studies should focus on the evaluation of nutritional interventions aimed at maintaining the nutritional status and muscle mass in these individuals.

Anthropometric indicators as a screening tool for sarcopenia in older adults from Florianópolis, Santa Catarina: EpiFloripa Ageing study

ABSTRACT Objective This cross-sectional population-based study aimed to determine the cutoff points of anthropometric indicators to screen for sarcopenia and the association between sarcopenia and these indicators. Methods The sample consisted of 601 older adults. Sarcopenia was detected by appendicular skeletal muscle index. The receiver operating characteristic curve identified the cutoff points for body mass index, waist-to-height ratio, waist circumference, and handgrip strength to screen for sarcopenia. Based on the cut-off points, principal component analysis determined which indicators had a better fit as discriminants to compose an indicator. Finally, logistic regression analysis was performed with this indicator as the main independent variable. Results The results showed that all anthropometric indicators and handgrip strength were capable of discriminating sarcopenia; however, body mass index, waist circumference, and waist-to-height ratio were the best discriminants in principal components analysis, composing the new indicator. Adjusted logistic regression analysis showed that sarcopenia was associated with the indicator in both sexes. Conclusion The concomitant presence of low body mass index, low waist circumference, and low waist-to-height ratio may help to discriminate sarcopenia.

Factors associated with diet quality of older adults

ABSTRACT Objective Evaluate the factors associated with diet quality of older adults from the city of São Caetano do Sul, São Paulo, Brazil. Methods A cross-sectional study was carried out on a sample of 295 older adults receiving care in health care units in São Caetano do Sul. Diet quality was assessed using the Brazilian Healthy Eating Index. The associations between the diet quality mean score and the socio-demographic, economic, and anthropometric characteristics and lifestyle and health conditions were verified using multiple linear regression. Results Lower diet quality mean score were associated with the variables: marital status (widowed or separated) (β=-2.02; p=0.047), retired (β=-4.24; p=0.034), and smoking (β=-8.06; p=0.001); whereas higher diet quality mean score were associated with higher education level (9 years or more) (β=3.49; p=0.013). Conclusion Individuals with higher education level had better diet quality, and those who were widowed or separated, retired, and smokers had worse diet quality indicating that socio-demographic, economic, and lifestyle are factors that can influence food choice of older adults.

Cross-cultural adaptation and content validation into Portuguese of the Subjective Global Nutritional Assessment for pediatric patients hospitalized with cancer

ABSTRACT Objective Cross-cultural adaptation and content validation of the Brazilian Portuguese version of the Subjective Global Nutritional Assessment questionnaire (originally in English) for use in hospitalized children and adolescents being treated in a reference institute of oncology. Methods The cross-cultural adaptation process consisted of the following stages: conceptual, item, semantic, and operational equivalence. The conceptual equivalence and item was carried out through discussion with members of an expert committee. Semantic equivalence was evaluated through initial translation, synthesis of translations, back translation, discussions with experts, and pretest with 32 patients. During operational equivalence, the experts discussed about the format of questions and instructions, setting, target populations, and mode of administration to later propose a final version. Content validation was performed by the expert committee. Results Minor modifications were made in the instrument to facilitate its use in the Brazilian socio-cultural context. Pretest results showed that the instrument is easily understood by health care professionals and the target population. Conclusion The cross-cultural adaptation and validation of the Subjective Global Nutritional Assessment allowed obtaining a Brazilian version equivalent to the original. The adapted instrument will be an important tool for the subjective assessment of the nutritional status of pediatric patients hospitalized with cancer.
