Suffering and defense mechanisms: an analysis of the work of Primary Health Care nurses

ABSTRACT Objective: To analyze the suffering and coping strategies of nurses working in Primary Health Care considering the psychodynamics of their work. Methods: Descriptive study with a qualitative approach, carried out with 11 nurses working in Primary Health Care. Data were collected through semi-structured interviews, systematized, and interpreted using Bardin’s content analysis method applied to recurrent themes. Results: The suffering factors related to work in Primary Health Care are difficulties with management, the fragile structure of the health support network, and conflicts with the users. Defensive strategies to minimize these difficulties are the support of hierarchical superiors, the empowerment of the community and users, and communication between team members. Final considerations: It is important that there be changes in the organization of this line of work to improve the workers’ health.

Reasons assigned to suicide attempts: adolescents’ perceptions

ABSTRACT Objective: identify the reasons for attempting suicide from the perspective of adolescents. Methods: qualitative study conducted with ten adolescents who attempted suicide and were attending a Centro de Atenção Psicossocial Infanto-Juvenil located in a city in the south of Brazil. Semi-structured interviews were held in July 2020 using WhatsApp. Data were analyzed according to Minayo’s Content Thematic Analysis. Results: the adolescents’ reports listed the reasons that triggered suicide attempts, such as changes in the adolescents’ life cycle and violence, which led them to attempt suicide to solve problems. Final considerations: Data analysis revealed the reasons that triggered suicide attempts from the adolescents’ perspective and difficulties to cope with problems, probably explained by their lack of experience in dealing with frustrations and disappointments.

Mental health assistance in Primary Care: the perspective of professionals from the Family Health Strategy

ABSTRACT Objective: To learn about the perceptions of the professionals who work in Primary Health Care about mental health care. Methods: Descriptive and qualitative study, carried out with 29 health workers through open and individual interviews. The IRaMuTeQ® software was used to organize the data which, then, was submitted to a content analysis process in the thematic modality. Results: Three classes emerged from the content analysis: “Perceptions about the mental health care provided in the city”, “The biomedical paradigm in mental healthcare”, and “Elements for the construction of a new way for professionals to act in mental health”. Final considerations: It was found that, despite the good infrastructure of the services and the elements pointed at by the professionals to create a new way to act, they do not do so, and the responsibility falls, mostly, on the psychologist to carry out these activities.

Religiosity and spirituality in mental health: nurses’ training, knowledge and practices

ABSTRACT Objective: to analyze nurses’ training, knowledge and practices in the religiosity/spirituality interface in the context of Psychosocial Care Centers. Methods: a descriptive, exploratory, qualitative study, carried out in a metropolitan region of Ceará. Four nurses were interviewed, and the data were examined using the content analysis technique. Results: four categories emerged from analysis: Deficit in teaching religious and spiritual practices in higher education in nursing; View of the concept of religiosity and spirituality; Religiousness and spirituality as an integral part of the person; Reception of religious and spiritual practices as an aid to treatment. Final considerations: a deficit in nursing undergraduate teaching was identified concerning religiosity and spirituality. However, the interviewed professionals understand the importance of religious and spiritual dimensions in the therapeutic process, even though they do not accurately differentiate the concepts.

Factors associated with suicide ideation of healthcare university students

ABSTRACT Objectives: to analyze the factors associated with suicidal ideation in students from healthcare graduation courses. Methods: quantitative, cross-sectional study, with 251 students from the courses of radiology, speech-language therapy, medicine, nutrition, health service management, and nursing, from a federal higher education institution in the Southeast of Brazil. Data were collected from August to October 2019, using an electronic questionnaire. Results: the prevalence of suicidal ideation among participants was 26.33%. In the final logistic regression model, only depressive symptoms were associated with suicidal ideation. Having symptoms of depression increased the chances of suicidal ideation 2.6 times. Conclusion: the high prevalence of suicidal ideation and its associated factors constitutes a situational diagnosis that demands the elaboration of public and institutional policies, focused on the promotion and attention to the mental health of the students.

Parental burnout: a scoping review

ABSTRACT Objective: to map available evidence on parental burnout theme. Method: scoping review as according to the Joanna Briggs Institute. Search in January 2021, in six databases of publications in English, Portuguese or Spanish, without time limits. Data extracted and descriptively analyzed by three independent researchers. Results: 374 articles were identified and 20 were included in the final sample, all published in English, from 2017 onwards. Parental burnout is a complex, multifactorial problem, distinct from burnout cases, depressive symptoms and other mental health alterations. It affects 0.2 to 20% of parents, has validated and suitable instruments for measurement; if not treated, can have consequences on marital life, work relationships, child neglect and violence. Conclusions: this is a recent theme that needs to be explored, due to the possible impact on children’s and families’ health and on parents’ work processes. Home care for children with gastrostomy

ABSTRACT Objectives: to describe home care practices performed by family members for maintaining the life of children with gastrostomy. Methods: qualitative research using the Sensitive Creative method, with the Criatividade e Sensibilidade Corpo Saber [Corpo Saber Creativity and Sensitivity] dynamics. The participation included ten family caregivers of children with gastrostomy. The outpatient clinic of a federal hospital in Rio de Janeiro was the setting. Lexical analysis was used through the IRaMuTeQ® software. Results: Theme 1, entitled “The care for maintaining life performed by family members of children with gastrostomy at home”, was analyzed, comprising three classes: “The gastrostomy tube home care routine”; “The care with the gastrostomy/stoma”; and “Food and medication home care routine of children with gastrostomy”. Final Considerations: family caregivers used strategies to maintain the device and acquired new knowledge in this field, and in nursing competence, regarding stoma care, food administration, medication, and syringes.

Nursing care for the Amazon population: knowledge production and human resource development

ABSTRACT Objectives: to analyze knowledge production adherence from a master’s course in nursing in the Amazon to care and health demands in the region, with an emphasis on neglected tropical diseases and traditional populations in the Amazon. Methods: a descriptive study, with a quantitative approach and documentary basis, analyzing dissertations defended in a Graduate Program in Nursing at the Universidade do Estado do Pará in association with the Universidade Federal do Amazonas, from 2012-2019. A descriptive statistical analysis was performed. Results: of the 105 dissertations analyzed, 30 (28.6%) were related to neglected tropical diseases. Of these, 11 (10.5%) dealt with Chagas disease, leishmaniasis, tuberculosis, malaria, and leprosy. Traditional populations in the Amazon participated in only 11 (10.5%) studies. Conclusions: knowledge production in a master’s course has adherence to care and health demands in the region; however, it needs to enhance its production to strengthen its identity.

Religious/spiritual coping in informal caregivers of children with cleft lip and/or dysphagic palate

ABSTRACT Objectives: to identify the use of religious/spiritual coping in informal caregivers of children with cleft lip and/or palate, dysphagic, powered exclusively by probe. Methods: descriptive and cross-sectional study, including 30 informal caregivers. For data collection, a Sociodemographic Questionnaire and the Brief Religious/Spiritual Coping Scale were used. For statistical analysis, ANOVA, Student’s t and Spearman’s correlation tests were used, with a significance level of 5%. Results: the use of religious/spiritual coping was high among participants (mean=3.71), with a predominance of positive (mean=3.30) compared to negative (mean=1.88). It was evident that the greater the reported importance of religiosity/spirituality in the participants’ lives, the greater the positive coping (p=0.001). Conclusions: informal caregivers used positive religious/spiritual coping as a way of coping with care demands related to the child’s health condition. These findings point to the importance of including spirituality/religiosity as health indicators.

Effect of educational video on newborn care for the knowledge of pregnant and postpartum women and their families

ABSTRACT Objectives: to evaluate the effect of educational video on newborn care to increase the knowledge of pregnant, postpartum, and family members. Methods: a quasi-experimental study, with pre-intervention and post-intervention evaluation with a single group. Fifty-eight pregnant, postpartum, and family members treated in basic health units and a hospital in Ceará, Brazil, participated. The study used the McNemar and binomial tests for the analysis. Results: after the intervention, there was an increase in the frequency of hits, from 70.82% to 92.97%. Most of the questions presented a significant increase of hits (p < 0.05) with an emphasis on sleeping position, drying of clothes, free demand for breastfeeding, and things to avoid (such as accessories in the sleeping place and talc in diaper change). Conclusions: the educational video was effective to participants in acquiring knowledge on the care of newborns and can assist in health education activities carried out by nurses.

Healthcare: action research with trans people living on the streets

ABSTRACT Objectives: to analyze the representations of healthcare provided to trans people living on the streets. Methods: ten women (three trans) and three cisgender men participated in this action research. Popular health education groups, focus groups, seminars and interviews were held, the data of which were organized in the software Nvivo®, submitted to content analysis and interpreted in the light of intersectionality theory. Results: healthcare was represented by the technical, relational, structural and citizenship dimensions. The relationship between gender and poverty determines the specific health needs of trans people living on the streets. Final Considerations: the need to expand conceptions and practices on healthcare to meet the specific health needs of trans people living on the streets was evidenced. Nursing, with competence and cultural sensitivity, can contribute to positive health outcomes and, consequently, break with the logic of exclusion, illness and poverty.

Content validation to support the monitoring of disclosure of HIV diagnosis in childhood

ABSTRACT Objectives: to create and validate the content of a guide for monitoring the communication of the HIV diagnosis in childhood. Methods: methodological study, with a design guided by the Knowledge-to-Action (KTA) Framework, supported by a participatory approach. The guide’s content was structured according to the communication elements proposed by Lasswel from review studies. Results: the content was validated by 26 experts from nursing, medicine, psychology and pedagogy, using a Likert-type scale for relevance, clarity and precision. Data collection took place online and achieved a Content Validity Index of 0.94. Conclusions: the guide can contribute to the practice of professionals who care for children living with HIV, to support the family in communication and to the child’s right to know their diagnosis. For further research, it is recommended to create and validate the face of the technology in order to implement it.

Adolescents, poverty areas, violence, and public health: an intersectional perspective

ABSTRACT Objectives: to discuss the influence of urban poverty on the context of violence among adolescents from an intersectional perspective. Methods: the original research, of the action research type, analyzed data from 13 workshops. The participants were adolescents from both sexes, from 15 to 17 years old, from a public school in a peripheral neighborhood of São Paulo, SP. The methodological proposition of intersectional analysis guided the interpretation of the empirical material. Results: the intersection of class and gender may increase the (re)production of violence in some men. The intersection of race/color, social class, and territory contributes to the construction of narratives that naturalize inequality and, thus, justify discrimination. Final Considerations: there is necessity of new public policies that consider the social contexts and experiences of the subjects that stem from the articulation of social markers.

Hospital structure elements demarcating (in)visibilities of institutional violence against children

ABSTRACT Objectives: to analyze the hospital structure elements that demarcate (in)visibilities of institutional violence in hospitalized children. Methods: this is a descriptive-exploratory qualitative study that used approaches with Foucault’s thinking. Ten companions and 39 healthcare professionals from a university hospital in Salvador, Bahia participated. Data collection took place from November 2018 to June 2019 through semi-structured interviews. The discourse analysis method was used. The study was approved by the Institutional Review Board. Results: institutional violence was understood in the violations and invisibilities of the structure of health services through the problems: in infrastructure (physical structure, lack of human and material resources, scrapping of equipment); administrative and management; pilgrimage. Final Considerations: it is necessary to realize the invisibilities of the infrastructure to act in confronting institutional violence to hospitalized children.

Knowledge, attitudes and practices related to sexually transmitted infections of men in prison

ABSTRACT Objectives: to analyze the knowledge, attitudes, and practices related to sexually transmitted infections of men in prison. Methods: qualitative study, based on the methodology Knowledge, Attitude and Practice, conducted with 30 men in prison. The study applied individual interviews submitted to the analyze of the discourse. Results: the incipient knowledge of men was associated with: self-illness and the others’, diagnosis, and perception of risk behavior, pervaded by distrust or lack of understanding of the rapid test for detection. The attitudes involved culpability of partnerships, non-adherence to prevention inputs, and resistance to look for health services. The practices are linked to health care after the identification of the disease, use of penile condoms, and attention to the partnership. Conclusions: the reduced knowledge about Sexually Transmitted Infections makes attitudes stereotyped, stigmatized, compromised by the level of education. The level of health care and the limits of deprivation of liberty weaken the practices.

Inter-sectorial network for assisting women in situations of violence: handicraft work built by the people

ABSTRACT Objectives: to discuss the meanings attributed by professionals to the construction of the service network for women in situations of violence. Methods: Convergent Care Research developed with the application of a convergence group with 32 participants from the health, social assistance, or public security sectors, in Santa Maria, Rio Grande do Sul. The data was produced during ten meetings, based on the problematization methodology, and submitted to analysis of thematic content and interpreted with the referential of networks. Results: the construction of the network is an artisanal work carried out by people continuously, permeated by bonds, communication and partnerships between services and knowledge of work processes. Lashings require permanent movement. Final Considerations: building a network implies defining intertwining lines; form nodes that create cross-sector connections. There is a need to institutionalize processes through care continuity protocols, associated with an efficient communication system between sectors.

Health beliefs and adherence of the elderly to fall prevention measures: a quasi-experimental study

ABSTRACT Objective: To evaluate the effect of an educational intervention on health beliefs and adherence of elderly people to fall prevention measures. Methods: This is a quasi-experimental study, carried out at the Senior Citizens' Center. Sixty-eight elderly completed the intervention. The intervention consisted of four meetings focused on beliefs about falls and prevention, and the evaluation occurred at baseline and 30 days after completion. Results: The elderly were predominantly women (83.82%), with one to four years of schooling (36.76%), with health problems (95.59%), and 48.53% had fallen. There was a significant increase in perceived susceptibility, severity, benefits, barriers, and total health belief score post educational intervention. By adding total to partial adherence, there was a significant increase in the adherence of the elderly to fall prevention measures after the educational intervention. Conclusion: Educational intervention was able to improve the beliefs and adherence of the elderly to fall prevention measures.

Vulnerability and quality of life of older persons in the community in different situations of family care

ABSTRACT Objective: to correlate socio-contextual aspects, physical vulnerability and quality of life of older persons in the community in different situations of family care. Methods: epidemiological, a cross-sectional and analytical study, with elderly people in the community (n=769), with application of the instruments: Vulnerable Elders Survey-13 (VES-13), World Health Organization Quality of Life for Older Persons (WHOQOL-OLD) and the World Health Organization Quality of Life (WHOQOL-BREF); and socio-contextual data questionnaire. Results: the population assessed presented an average of regular quality of life in both the WHOQOL-BREF and the WHOQOL-OLD. Older non-vulnerable persons (62.2%) and those with close family contact (82.6%) have a better quality of life than the vulnerable (p<0.0001). Conclusion: lower quality of life scores and more distant families are related to vulnerable elderly people; thus, the assessment of family proximity and physical vulnerability of older persons is shown to be an important factor in improving quality of life.

Transitional care of nurses to older adults with artificial pacemaker

ABSTRACT Objective: to understand how the transitional care of nurses to olde adults with artificial pacemaker occurs. Method: a qualitative research, carried out in a philanthropic hospital in the city of Salvador, Bahia, Brazil. Fourteen nurses working in surgical and cardiac inpatient units participated. Data were collected between August and November 2020, through semi-structured interviews, and analyzed using the methodological framework Discourse of the Collective Subject and discussed in the light of Afaf Meleis’ theory of transitions. Results: nurses’ incipient knowledge about transitional care was identified. It was observed that the nursing discharge report is a facilitating instrument for transitional care. Final considerations: the study pointed out that the transitional care of nurses to older adults with artificial pacemakers does not have a theoretical foundation, reporting as a priority the care with the pacemaker identification card and with the surgical wound.
