Scenario validation for clinical simulation: prenatal nursing consultation for adolescents

ABSTRACT Objectives: to validate a scenario for clinical simulation: prenatal nursing consultation for adolescents. Methods: methodological study developed from January to December 2019, in five stages (overview, scenario, scenario design, progression, debriefing and assessment). The validation involved four volunteer students, a teacher as a facilitator and four judges. The judges filled out a Likert scale with four responses. Data was analyzed using Microsoft Excel® software, version 2016. Absolute and relative frequencies and the content validity index were calculated, considering a minimum acceptable value of 1.0. Results: the preparation of the scenario was based on the proposed learning objectives. The scenario was validated with a global content validity index equal to 1.0. Final Considerations: the study achieved the proposed objective. This scenario can contribute to preparing nurses to work in the care of pregnant teenagers, a representative public in Brazil that requires specific care.

Nurses’ stress and resilience before and after evaluation for hospital accreditation

ABSTRACT Objectives: to analyze and compare levels of stress and resilience in nurses before and after the assessment for maintenance of the Hospital Accreditation Certification. Methods: quantitative, observational, and longitudinal research, with 53 nurses from a philanthropic hospital, in the Rio Grande do Sul. Data collected in two stages, March, and July 2019, before the assessment visit and 60 days after, using the Bianchi Stress Scale and Resilience Scale. Descriptive and analytical statistics were employed. Results: the majority of participants showed an average stress level before and after the evaluation. The highest stress scores were related to Domains E (coordination of unit activities) and C (activities related to personnel administration). In both moments of the study, the participants had medium and high resilience. Conclusions: managing people, processes, and assistance are stressful activities in the Accreditation process and increase the nurses’ stress levels.

Urinary tract infection in patients with chronic kidney disease under conservative treatment

ABSTRACT Objectives: to assess the prevalence and associated risk factors for urinary tract infection in patients with chronic kidney disease under conservative treatment and identify the microorganisms isolated in the urine of these patients and the staging of chronic kidney disease. Methods: a cross-sectional, analytical study carried out at the Conservative Treatment Outpatient Clinic of a university hospital in the city of São Paulo. Results: the prevalence of urinary tract infection is 22%. The risk factors Diabetes Mellitus, hypertension, heart disease, neoplasms and thyroid and autoimmune diseases stand out in the infected group (p < 0.001). Most of the microorganisms found in urine cultures (87.9%) were Gram-negative, being Escherichia coli (50.70%), followed by Klebsiella pneumoniae (23.1%) and Enterococcus spp. (9.7%). Conclusions: the findings of this investigation reveal the intrinsic association between risk factors and microorganisms for the development of urinary tract infection.

Factors associated with bronchopulmonary aspiration: a national-based study

ABSTRACT Objectives: to determine the prevalence of bronchopulmonary aspiration in the Brazilian scenario, the factors associated with the incident and the variables associated with death. Methods: a cross-sectional and analytical study, carried out from analysis of notifications of incidents related to bronchopulmonary aspiration of the Health Surveillance Notification System, from January 2014 to December 2018. Results: of the 264,590 notifications, 553 referred to aspiration, whose prevalence rate was 0.21%. There was an association between the event and age, ethnicity, main medical diagnosis, country region, service type, health unit and consequences for patients. Furthermore, four independent predictor variables for death were found: living in the North or South regions, being elderly and receiving healthcare at night. Conclusions: the prevalence rate of bronchopulmonary aspiration was small, but with a negative impact on patients.

Undernutrition and associated factors in primary schoolchildren in Lokossa, Benin: a cross-sectional study

ABSTRACT Objectives: to estimate the prevalence of stunting and thinness and sociodemographic, household, family, dietary, and scholar associated factors in schoolchildren living in Lokossa-Benin. Methods: a survey conducted in a probabilistic sample (n=615) of primary schoolchildren (8-17 years), from 12/2018 to 01/2019, using structured questionnaire and 24-hour dietary recall. Thinness and stunting were defined as Height-for-age and Body Mass Index-for-age below-2 standard deviations, respectively. Associations were estimated by chi-square test and logistic regression. Results: thinness and stunting prevalence was 13.1% (95%CI:9.0-18.7) and 25.5% (95%CI:20.6-31.2), respectively. Odds of thinness were higher among older schoolchildren and those who experienced hunger at school. Odds of stunting increased with age, low diet diversity, experiencing hunger at school, and having school meal five days a week (OR:2.09; 95%CI:1.29-3.36). Conclusions: stunting was the most common problem. Older schoolchildren and those with food deprivation or poor diet diversity were the most affected.

Unsatisfactory completeness of nurses’ records in the medical records of users with tuberculosis

ABSTRACT Objectives: to evaluate the completeness of nurses’ records on the execution of the nursing process in assistance of tuberculosis patients at Primary Care. Methods: this was a retrospective documental study, with 190 records in Family Health Units of a city in the state of Paraíba. The data were analyzed according to descriptive statistics, Pareto Diagram, and trend analysis. Results: the overall mean incompleteness of records was 53.01% (DP=26.13). Therefore, the results presented very poor completeness classification related to nursing diagnosis (88.9%), nursing assessment (66.8%), data collection (60.5%), while nursing interventions were classified as regular (11.1%). The nursing diagnosis was the only variable with a decreasing trend of non-completeness. Conclusions: incompleteness of nurses’ records in the medical records of users with tuberculosis. Evaluation strategies, permanent and continuing education are indispensable in the quality of nurses’ documentation, directly implying the Systematization of Quality in Nursing Care.

Knowledge of caregivers and factors associated with neuropsychomotor development in children

ABSTRACT Objectives: to analyze the knowledge of caregivers and the factors associated with neuropsychomotor development in children. Methods: a cross-sectional study, with a quantitative approach, was conducted with 220 child-caregiver binomials attended in the public health services of the municipality of Parnaíba, State of Piauí. The study used the “Denver Test II” and the “Inventory of Child Development Knowledge”. Results: the study classified 197 children with natural development and 23 with suspicious development. Caregivers with a higher level of knowledge about child development were associated with children with better development. The study considered caregiver knowledge, gestational age, and exclusive breastfeeding as protective factors for appropriate neuropsychomotor development when used the regression model. Conclusions: actions aimed at indicators that presented positive associations must be implemented to improve child development, such as educational activities to increase the level of knowledge of caregivers, improvement of prenatal monitoring, and encouragement of breastfeeding.

Professional performance in the administration of medicines in pediatrics: a study cross-sectional observational

ABSTRACT Objectives: to evaluate Patient Safety actions carried out by nurses in the Administration of Medicines in Pediatrics. Methods: observational, cross-sectional, quantitative study carried out in 2018, with 25 nurses from a Hospital in Peru. 183 observations were made; the instrument Safety of the patient was used in the administration of medicines in Pediatrics Spanish version. The performance of the professionals was evaluated by the Positivity Index. Results: of 22 itens observed, five were adequate, five were safe, two were classified as borderline and ten as endurable among those who stand out; Bring to bed only medications prescribed for a single child and maintains an adequate record of prepared medications that will be stored. Conclusions: weaknesses were observed in the medication administration process, since only 10 itens were classified as safe or adequate, reinforcing the need for permanent education activities by the institution for nursing training.

Postgraduate nursing students’ conceptions on scientific integrity and research ethics

ABSTRACT Objectives: to understand the conceptions of postgraduate Nursing students regarding integrity in scientific research. Methods: a qualitative, descriptive and exploratory study conducted in October 2020, with 40 students from a postgraduate program in Nursing from a public university in Southern Brazil. The interviews had an emphasis on scientific integrity and its importance for scientific research; they were conducted using synchronous communication tool and analyzed by textual discourse analysis. Results: two categories emerged: “Scientific integrity: the ethical dimension of research” and “Implications of academic productivism for scientific integrity”. Such integrity is intrinsically related to the supporting ethical elements, as well as to the nuances of productivism and its impacts on integrity. Final Considerations: the cross-sectionalism of ethical practices is necessary in the production spheres of science, so that knowledge is perpetuated in harmony with integrity and its representations to the community.

Supervised Internship: attributions and limitations from the perspective of nursing supervisors, faculty advisor and managers

ABSTRACT Objectives: to describe the attributions and limitations in internship nursing supervisors’, faculty advisors’ and managers’ work in the process of carrying out Supervised Internship in nursing education. Methods: a qualitative exploratory-descriptive research. Interviews were conducted between February and July 2018 with 26 participants: four managers, nine faculty advisors, six primary care nursing supervisors and seven hospital care nursing supervisors. Data analysis was performed using the Content Analysis technique. Results: among the main attributions are participation in the organization of the field, mediation between educational and health institutions, presentation of the world of work; as limitations are the high number of institutions in the field, work overload, lack of commitment or lack of didactic preparation towards student education. Final Considerations: we evidenced the need to (re)define and clarify everyone’s roles and (re)think the integration strategies for monitoring students.

The influence of nursing team’s behavior in adverse event following immunization surveillance

ABSTRACT Objectives: to analyze the effects of nursing professionals’ behavior in adverse event following immunization surveillance. Methods: a cross-sectional study of 384 participants who received vaccines. Information on vaccination history, administered vaccines and vaccination guidelines were analyzed. Descriptive and bivariate analyzes were performed using simple logistic regression (unadjusted Odds Ratio). Results: guidelines on events (PR=1.8; p=0.001) and conducts regarding their occurrence (PR=1.7; p=0.001) are activities that influence adverse event following immunization surveillance. More than half of participants did not receive guidance on the vaccines administered, the events and the conduct in case of an occurrence. Only 38.5% were instructed about the vaccines administered and 40.6% about adverse events. In the presence of an event, 29.9% reported that they sought services for notification. Conclusions: proper screening, providing guidance on vaccines and adverse events are essential preventive measures to strengthen adverse event following immunization surveillance.

The Healthcare Failure Mode and Effect Analysis as a tool to evaluate care protocols

ABSTRACT Objectives: to identify, classify, and analyze modes of failure in the medication process. Methods: evaluative research that used the Healthcare Failure Mode and Effect Analysis (HFMEA) in a service of bone marrow transplant from June to September 2018, with the participation of 35 health workers. Results: 207 modes of failure were identified and classified as mistakes in verification (14%), scheduling (25.6%), administration (29%), dilution (16.4%), prescription (2.4%), and identification (12.6%). The analysis of risk showed a moderate (51.7%) and high (30.9%) need of intervention, leading to the creation of an internal quality assurance group and of continued education activities. Conclusions: the Healthcare Failure Mode and Effect Analysis showed itself to be a tool to actively identify, classify, and analyze failures in the process of medication, contributing for the proposal of actions aimed at patient safety.

Nurse’s competences in promoting women’s health in light of the Galway Consensus

ABSTRACT Objectives: to identify nurses’ competences in promoting women’s health. Methods: descriptive study with a qualitative approach that adopted the Galway Consensus as a theoretical-methodological framework. Data collection was performed through semi-structured interviews. Results: most of the Galway Consensus’ competence domains were present in the nurses’ interventions related to health promotion in women’s care. “Assessment of needs” and “Implementation” were the most highlighted domains, followed by “Leadership” and “Impact assessment”. The domain “Defending/Advocating Rights” was not identified. Final Considerations: within the nurses’ work with women, some of the Galway Consensus domains of competencies for health promotion were present. However, there is a need, in the context of continuous health education, to expand strategies and enhance the development and application of these health promotion competences.

Educational needs in palliative care of Primary Health Care nurses

ABSTRACT Objectives: to understand the experience of Primary Health Care nurses with palliative care and their educational needs on the topic. Methods: an exploratory study, with a qualitative approach. The data was collected through 19 focus groups involving 181 nurses. In interpreting the results, the thematic analysis was used. Results: the knowledge presented by nurses in palliative care proved to be limited. Gaps were identified in the definition of palliative care and in the philosophical principles that guide it. The experiences described highlighted the need for continuing education regarding the definition and philosophy of palliative care, patient eligibility, integral care to the patient and the family, interpersonal communication and difficult news, performance in a multi-professional team, and end-of-life care. Final Considerations: the analysis of the perceptions of Primary Health Care nurses about palliative care can help identify the obstacles to improving the provision of this care modality.

Nurse training for care management: integrative literature review

ABSTRACT Objectives: to identify how nurses are trained to manage care. Methods: an integrative review, guided by the research question: “How does the training of nurses for care management occur?” The studied performed the search on LILACS, BDENF, CINAHL, Cochrane Library, BVS, and SciELO, using combined strategies to select publications indexed in databases and electronic libraries. The analysis involved bibliometric aspects and the evidence on nursing education contained in the articles. The final sample consisted of eight articles. Results: three thematic categories emerged: conceptions and senses; fragmentation of care; and training practices. They showed essential aspects about the nurse training process for care management. Final Considerations: research needs to address conceptions inherent to teaching processes focusing on concepts, significance, and application, engaging the development of macro skills to transpose care beyond theory.

Use of technology for self-care in surgical wound infection surveillance: integrative review

ABSTRACT Objectives: to synthesize knowledge about technology for self-care in surgical wound infection surveillance. Methods: integrative review conducted in CINAHL, Embase, LILACS, PubMed, Scopus and Web of Science databases with the descriptors “surgical wound infection” and “self-care”, in addition to manual search in the references of the included studies. The selection and evaluation of the methodological quality of the studies and data collection were performed by two independent reviewers; conflicts were resolved by a third reviewer. Results: nine primary studies were included, published between 2011 and 2019. Six are cross-sectional, two are randomized clinical trials, and one is a case report. Mobile, text messaging, and imaging applications, computer software, assessments, and data storage capacity stand out. Conclusions: the technologies identified for self-care in surgical wound infection surveillance were the creation and use of mHealth and the use of health apps on mobile devices. Effective technologies in surveillance of surgical wound infection that enable rapprochement with the healthcare team, encourage a greater number of surgical wound assessments, enhance self-care actions, and decrease patient anxiety. Technology is also a monitored and recorded form of patient care, one of the main axes of infection surveillance.

Storage of gastrointestinal endoscopes: when is the safe time for re-use?

ABSTRACT Objectives: to identify the safe storage time for the use of flexible gastrointestinal endoscopes after high-level disinfection, as well as the defining criteria for this time. Methods: an integrative literature review was carried out in the Virtual Health Library, PubMed, Scopus, and Web of Science, considering original articles published since 2000. Results: eleven articles were selected, whose storage times ranged from 1 to 56 days, with a predominance of one to seven days (73%). Several criteria were used to define this time, predominantly the premise of efficient processing (100%), use of alcohol flush (64%), use of drying cabinets (18%), among others. Conclusions: the criteria for determining the storage time did not show a consensus for clinical practice. Expanding the discussion of this theme with the definition of the minimum necessary conditions is of fundamental importance for the reduction of risks and safety of the procedure and the patient.

Involvement of companions in patient safety in pediatric and neonatal units: scope review

ABSTRACT Objectives: to describe scientific evidence on the involvement of companions in patient safety, from their own perspective and health professionals’ perspective in neonatal and pediatric units. Methods: scoping review carried out according to The Joanna Briggs Institute’s recommendations, in eight databases, following the Preferred Reporting Items checklist for Systematic Reviews and Meta-Analyses extension for Scoping Reviews checklist, between 2011 and 2021. Results: the 13 studies included highlighted the importance of companions’ involvement in patient safety and the prevention of adverse events. However, they pointed out failures in communication and weakness in the training of professionals, which were obstacles to their involvement. The strengthening of health education, multidisciplinary rounds and educational technologies were highlighted as strategies to expand the involvement of companions. Final Considerations: this study directs elements for health professionals and managers to rethink the companions’ role in patient safety and development of collective strategies.

Needs, parenting practices, and dissemination of information on social-emotional skills and development of infants

ABSTRACT Objectives: to describe the development of a technological innovation designed to identify needs, parental practices, and disseminate information on social-emotional skills and development of infants from the perspective of family members of infants on the social-emotional development of children. Methods: a report on the development of an innovation consisting of an educational group and a card game on social-emotional skills and development of infants. A spider-web concept map, content analysis and the Bioecological Theory of Human Development as a theoretical reference were used for data analysis. Results: the educational group and card game enabled the identification of information needs on child behavior and playing, and parenting practices focused on affectionate interactions, responsive caregiving, and socialization. Conversation with experienced parents, educational groups, apps, and health consultations were desirable dissemination strategies. Conclusions: the educational group and the card game represent an innovative, low-cost technology with potential for large-scale application in primary healthcare services.

Social determinants of health that permeate the mental suffering of children on the french-brazilian border

ABSTRACT Objectives: to understand the determinants involved in the mental suffering of schoolchildren on the French-Brazilian border. Methods: a qualitative study was conducted with children from 6 to 12 years of age from four State schools in the municipality of Oiapoque, located on the French-Brazilian border. The data were obtained through interviews, analyzed by the IRAMUTEQ software, and interpreted in the light of the Dahlgren and Whitehead’s social determinants of health model. Results: data indicate that the mental suffering experienced by children, especially characterized by anxious symptoms, such as concern of becoming fat and worries, were permeated by social situations that children went through daily, such as family abandonment, domestic violence, and bullying. Final Considerations: social determinants can be predictors of mental suffering in children, especially in environments of extreme vulnerability, such as the border, in a context of financial difficulties, domestic violence, and absence of parenting.
