Objectives: To compare definitions of high disease activity of the Ankylosing Spondylitis Disease Activity Score (ASDAS) and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) in selecting patients for treatment with biologic disease-modifying antirheumatic drugs (bDMARDs). Methods: Patients from Rheumatic Diseases Portuguese Register (Reuma.pt) with a clinical diagnosis of axial spondyloarthritis (axSpA) were included. Four subgroups (cross-tabulation between ASDAS (≥2.1) and BASDAI (≥4) definitions of high disease activity) were compared regarding baseline characteristics and response to bDMARDs at 3 and 6 months estimated in multivariable regression models. Results: Of the 594 patients included, the majority (82%) had both BASDAI≥4 and ASDAS ≥2.1. The frequency of ASDAS ≥2.1, if BASDAI<4 was much larger than the opposite (ie, ASDAS <2.1, if BASDAI≥4): 62% vs 0.8%. Compared to patients fulfilling both definitions, those with ASDAS ≥2.1 only were more likely to be male (77% vs 51%), human leucocyte antigen B27 positive (79% vs 65%) and have a higher C reactive protein (2.9 (SD 3.5) vs 2.1 (2.9)). Among bDMARD-treated patients (n=359), responses across subgroups were globally overlapping, except for the most ‘stringent’ outcomes. Patients captured only by ASDAS responded better compared to patients fulfilling both definitions (eg, ASDAS inactive disease at 3 months: 61% vs 25% and at 6 months: 42% vs 25%). Conclusion: The ASDAS definition of high disease activity is more inclusive than the BASDAI definition in selecting patients with axSpA for bDMARD treatment. The additionally ‘captured’ patients respond better and have higher likelihood of predictors thereof. These results support using ASDAS≥2.1 as a criterion for treatment decisions.

This paper is to assess the efficacy of different biologic DMARDs (bDMARDs) on several patient-reported outcomes (PROs) in randomized controlled trials (RCT) in axial spondyloarthritis (axSpA). A systematic literature review (SLR) was performed. MEDLINE (May 1, 2018) was used with the filters "published in the last 10 years" and "humans." The PICO criteria used were Patients: adults with radiographic axSpA (r-axSpA) or non-radiographic axSpA (nr-axSpA); Intervention: any bDMARD; Compararator: placebo (PBO)/any different drug; Outcome: the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), the Bath Ankylosing Spondylitis Functional Index (BASFI), the Ankylosing Spondylitis Quality of Life (ASQoL), the EuroQol-5D (EQ-5D), the Short Form 36 Health Survey physical component summary (SF36-PCS), the Short Form 36 Health Survey mental component summary (SF36-MCS), and the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F). After screening 84 initial references and manually selecting other 9, 24 publications, assessing TNF inhibitors (TNFi) or IL17A inhibitors (IL17Ai) were selected. Four RCTs quantified the minimal clinical important difference (MCID) between treatment arms. Most of the RCTs compared the mean difference of PROs between different timepoints. Overall, the treatment arm was superior to the comparator. PROs were often underreported or highly heterogeneously presented. MCID was seldom mentioned. There is a need to raise the standard of care on SpA by aiming at remission and PRO associated improvements. In order to achieve this goal, the target must be clearly defined, reported, and tested.

A rinoplasta de preservação é uma técnica descrita há bastante tempo e que recentemente tem ganho um crescente interesse por parte dos cirurgiões plásticos faciais. A preservação do dorso é uma parte fundamental deste procedimento. Duas técnicas principais serviram como base à sua evolução e desenvolvimento – a técnica “push down” e a “let down”. A preservação do dorso e a mobilização da estrutura osteocartilagínea como uma só unidade traz várias vantagens em relação à rinoplastia tradicional de resseção e reconstrução. Esta promete uma aparência melhor e mais natural, menos complicações e melhor funcionalidade a longo prazo. É uma técnica com grande potencial quando utilizada em doentes bem selecionados, de modo que se espera uma grande evolução nesta área nos próximos tempos.

Synovial tissue can be safely and reliably collected for research and clinical purposes using arthroscopy. This technique offers the obvious advantage of allowing direct visualization, and targeted biopsy of specific areas of interest within the joint, as well as for the collection of tissue which will include a lining layer. Much has been learnt by studying the synovium retrieved using this technique concerning the pathobiology of inflammatory arthritis. Furthermore, recent evidence suggests that the tissue retrieved may enable the identification of unique pathotypes that will allow for a precise approach to treatment selection in individual patients. Although ultrasound guided techniques for sampling synovial tissue have gained in popularity over the last decade, both methodologies are expected to compliment each other, each having unique benefits and drawbacks. We present here a detailed description of the arthroscopy technique reporting on our collective experience at two centers in Europe.

Neste estudo aborda-se o processo de criação de uma escola para o género feminino, na freguesia de Pereira do concelho de Ovar, oferecida por um empresário local. O benemérito era o empresário João Rodrigues de Oliveira Santos, regressado do Brasil e dono de uma fábrica de chapéus, naquela freguesia. O processo teve início em 1873, mas arrastouse por vários anos, envolvendo autoridades municipais e nacionais. O combate ao analfabetismo, em Portugal, envolvia esforços das autoridades políticas, mas mereceu o contributo de entidades e individualidades civis, abrindo escolas. A criação de escolas estava regulamentada e obedecia a um conjunto de princípios e requisitos formais, que haviam sido consignados por Portaria de 1866, quando da aceitação e execução do legado Conde Ferreira. No caso, em estudo, observa-se uma controvérsia entre os intervenientes, que assumiu contornos de disputa entre facções políticas, nos planos local e estatal, como comprovam fontes oficiosas e depoimentos da imprensa local.This study looks at the process of creating a school for female pupils in the parish of Pereira in the municipality of Ovar under the patronage of a local businessman. The benefactor was the local businessman João Rodrigues de Oliveira Santos, a hat factory owner returned from Brazil. The process began in 1873, but was held up for several years, with local and national authorities drawn into the debate. The fight against illiteracy in Portugal was led by the efforts of the political authorities, but received vital support from civil society organisations and individuals who opened new schools across the country. The creation of schools was regulated and subject to a set of principles and formal requirements introduced under the Ordinance of 1866, when the Conde Ferreira legacy was accepted and executed. This study examines the political controversy surrounding the attempt to open a school for girls in Pereira, as chronicled by unofficial sources and the local press of the time.

The relevance and lasting influence of artistic expressions of the philosophical concept of the absurd has been highlighted by many scholars, notably by Martin Esslin in his pioneering work The Theatre of the Absurd and by Neil Cornwell in The Absurd in Literature. Interestingly, a cursory glance at the works of the practitioners of the absurd deemed most significant by Esslin (Adamov, Beckett and Ionesco, but also Pinget, Pinter and Hildesheimer) and Cornwell (Daniil Kharms, O’Brien, Kafka and again Beckett) reveals an intriguing pattern: virtually all of them have at least one important work with older people as main characters. In this chapter, my main aim is to understand why old age is so prominent in absurdist literature. First, I will examine some of the most archetypal interactions between the aged protagonists and their absurd universes, refining my working definition of the absurd (wherein “resistance” is an operative word) in the process. Then, after assessing the congruity of outliers and the pertinence of alternative causal explanations for this correlation, I will demonstrate that the thematization of aging characters in absurdist works owes much to the fact that old age may be seen as the radicalization of the human condition.

It is well known that Alfred de Musset’s Lorenzaccio (1834) – which revolves around Lorenzo de’ Medici’s assassination of his cousin, the Duke of Florence, in 1537 – owes its existence in large part to Musset’s then lover George Sand; having previously written a shorter play on the same topic, she essentially bequeathed it to him, and he was to do with its subject matter and raw material as he pleased. However, it is by no means irrelevant to point out that, «[i]n the early 1830s, Alfred de Musset was fascinated by the Don Juan legend and by the nature of the man himself», which prompted him to write, in 1832 and 1833, three poems that «refer directly either to the Don Juan legend or to Mozart’s opera». Was this influence decisive in Musset’s shaping of ‘Lorenzino’ into a silver-tonged seducer? Or does Benedetto Varchi’s account of the historical Lorenzo, his contemporary, already lend itself naturally to such a portrayal? Which aspects of seduction were emphasised in the different adaptations of this story? This paper will attempt to address this last question, the answer to which could be a first step towards a diachronic analysis of the intricate theme of seduction in the most significant efforts to render the enigmatic figure of Lorenzino de’ Medici.

This article sets out to examine the aged radio body in Samuel Beckett’s All That Fall. Its main argument is that there is a conceptual symbiosis between the radiogenic problematisation of corporeality and the deliberate thematisation of old age in All That Fall. The two main issues explored in this article – which are inextricably intertwined – concern the notion of reluctant corporeality, that is, the idea that the radiophonic body in All That Fall is very real but also frail and therefore resented; and the understanding of old age as a crucial theme that can and should be interpreted in the light of different age theories, such as disengagement theory, irremediable regret, and the réveil mortel.

In this article, I endeavour to demonstrate how a number of works by like-minded mid-twentieth-century radio practitioners (Robert Pinget’s La Manivelle, Samuel Beckett’s Embers, Harold Pinter’s A Slight Ache and Tom Stoppard’s Artist Descending a Staircase) thematize old age the better to exploit (or perhaps even parody) radio’s perceived unique ability to foster epistemological scepticism and to enable the conceptualization of fantasies of non-embodiment. I argue that these works do not seek to establish an aural aesthetics of non-embodiment but instead remain explorations of (im)possibilities which are never even fully articulated, much less hankered after or dismissed offhand.

This study focuses on the analysis of a group of Latin and Greek inscriptions mostly coming from Ancient Latium. They are part of a manuscript file attached to a printed volume by Hesiod dating back to 1537. The volume is now kept in the ‘Biblioteca Vallicelliana’ of Rome. The book was part of the collection belonging to the Lusitan Aquiles Estaço, better known by the latinized name Achilles Statius, and thus he was identified as the author of the attached manuscript. However, for various reasons, his authorship has been questioned and this explains Hermann Dessau’s cautious choice to attribute the manuscript to an anonymous author.

The article takes an in-depth look into the sources concerning Augustus's childhood and the collocation of the suburban residence of his paternal grandfather, which Suetonius identifies as being iuxta Velitras. Considering the above-mentioned sources it cannot be excluded that this residence was at Ulubrae, a little town on the edge of the Pontine Marshes, near Velitrae, Cora and Norba, which Horace mentioned as desertissimus vicus. Ulubrae's bad reputation is due to a superficial reading of the sources and to the incredible persistence of the Horatian tòpos. However, the institutional town life reveals a certain growing livliness while its economy was mainly based on agriculture. Numerous indications would seem to confirm that the young C. Octavius, future Agustus, was nutritus in this secluded territory. Il presente articolo propone una lettura critica delle fonti sull'infanzia di Augusto e sulla collocazione dell'avito suburbano del nonno paterno che Svetonio colloca iuxta Velitras. Alla luce delle sopra menzionate fonti non si può escludere che tale residenza potesse trovarsi proprio presso Ulubrae, piccola città ai margini delle paludi pontine, confinante con Velitrae, Cora e Norba, menzionata da Orazio come desertissimus vicus. La pessima fama di Ulubrae si deve a una lettura superficiale delle fonti e alla incredibile persistenza del tòpos oraziano : nelle vicende istituzionali cittadine, infatti, si coglie una evoluzione che è sintomo di un certo dinamismo del piccolo centro la cui economia era prevalentemente incentrata sullo sfruttamento agricolo del territorio. Numerosi indizi sembrano confermare che proprio in questo recondito territorio fosse stato nutritus il giovane C. Ottavio, futuro Augusto.

This article analyses an apparently trivial but curious detail, that is to say, a small number of sardines being distributed to the sodales by the magister cenarum during the banquets of the collegium of Diana and Antinous. The translation of the epigraphic text does not rule out the possibility that it could be the distribution of one sardine to each diners, in addition to the bread and wine and other equipment established by the lex. The presence of these very cheap fish in such a modest quantity seems peculiar and it does not seem justified by the meal necessity. So, it can be supposed that the sardine could perform a more specific ritual function: it was probably used to fulfil an auspicious rite which cultores carried out at the beginning of the banquet. An analogy with the more articulated sacrifice in honour of the goddess Tacita Muta could be detected in this rite: the sacrifice, reminded by Ovid, was performed in order to hush up the slanderers, a circumstance which was very likely to happen at the rowdy banquets of the association.

The article presents an original inscribed and fragmentary grave slab, seen in Cisterna di Latina and coming from Bufaloreccia, nearby a big mausoleum with a cylindrical floor plan. The latter is already known to the archaeological bibliography and it is to be found alongside the Appian way. The evidence dates back to the second half of the 2nd century A.D. and it is ascribable to the ager of the Ulubrae municipium, a less important town on the slopes of the Lepini mountains (between Cora and Norba). The dedicatee is a Iulia whose condition is unknown as the evidence is too fragmented. However, it is worth noticing the recurrence of this gens in the Ulubran territory which some sources consider to be the original site of Augustus’ paternal family.

Mervyn Peake's most famous work, The Titus Books (also called The Gormenghast Series), has traditionally been read as a coming-of-age gothic fantasy story, laden with excentricities for decoration's sake alone. This thesis aims to argue that the author's refusal to "explain" his work should not stop the reader from looking for the meaningful patterns which make up the overarching theme of the series: tyranny. Centered around the topic of tyranny in its many forms — political, religious, familial, romantic — the story of Gormenghast and its heir Titus shows us how this authoritarian structure comes to be, what it does to those it subjugates, and what will happen to the few who attempt escape.

A representação da educação envolve uma noção sistémica e os intelectuais podem funcionar como sistema. Eles configuram e demarcam o campo educacional. O intelectual constitui um conceito que pode ser trabalhado enquanto categoria epistémica com educabilidade e historicidade. Os Intelectuais cedo percepcionam o que está ‘de novo’ em crise e o que deverá permanecer. Entre estes emergem os Escolares; combinando as dimensões de intelectualidade e educabilidade, são intelectuais envolvidos institucionalmente e que estabelecem o cruzamento entre educação e sociedade, são instituintes. Reconhecidos como mestres, são, em regra, autores de textos doutrinários e livros escolares; têm especial relevância na constituição do cânone escolar, literário, científico, artístico. Nada do que é humano lhes é indiferente; sabem fazer uso do poder simbólico e material que o institucional e o normativo lhes conferem. Será sobre os Escolares que procurarei desenvolver o meu texto. Tomarei como referência fundamental os intelectuais portugueses e a história da educação em Portugal.

O presente estudo sistematiza os processos seguidos na construção da denominada Escala de Avaliação da Autonomia Profissional dos Professores (EAP). Os resultados obtidos nos 32 itens desta escala foram analisados quanto à fidelidade e à validade de construto e externa. Uma análise factorial de componentes principais com rotação varimax revelou a existência de seis factores específicos, para além de um factor geral, relativos à multidimensionalidade do construto autonomia profissional dos professores. A variância explicada pelo conjunto dos seis factores atingiu 64.56% da variância total dos resultados. Os coeficientes de consistência interna mostraram-se muito elevados, para os diferentes factores e para variados grupos de pertença. Os índices de validade externa apresentaram-se muito satisfatórios. Os resultados mostraram-se de acordo com os posicionamentos teóricos existentes, quer acerca da autonomia profissional (Friedman, 1999) quer no âmbito da perspectiva psicométrica da avaliação psicológica.

Esta dissertação debruça-se sobre o conceito de fidelidade ontológica e a importância da questão ecológica para a filosofia de Cerqueira Gonçalves. O seu pensamento acerca da cultura, ética e ontologia depreende uma análise cuidada das razões de ser da insustentabilidade ambiental, não encontrando na ética e na política uma solução suficientemente satisfatória para esta questão. O filósofo defende, antes, uma reflexão profunda sobre a complexidade do real, sobre o lugar do homem no mundo, pela pergunta pelo ser, pela relação entre o uno e o múltiplo. Defende a primazia da ação e o papel fundamental da filosofia nas desejadas mudanças, adiantando que é a hermenêutica que oferece as ferramentas para a procura dos fundamentos que sustentam o modo de estar comunitário. Apresenta o amor e comunicabilidade como modos de ser partilhados por todos os entes, propondo um itinerário comunitário de fidelidade ontológica na relação triádica entre ser, homem e mundo.

No decurso do período moderno e com maior incidência no período contemporâneo, na sequência das revoluções liberais e republicanas e no quadro dos regimes democráticos actuais, os municípios constituíram e constituem instância de representação e desenvolvimento. São formados por população, território, poder administrativo e capacidade económico-financeira. Comportam poder de decisão. No campo da educação, são instâncias com legitimidade meta-educativa, nomeadamente em matéria de costumes, cidadania, consulta e decisão, oferta escolar. São constitutivos da memória colectiva e de identidade social e individual. Este Dossier temático da Revista Sarmiento incide sobre Municípios e Educação, numa perspectiva histórico-pedagógica, transversal a diferentes países, mas centrada no município, associado aos planos local e regional. Nasce deste olhar uma universalidade proporcionada pelos Estados-Nação na construção do Ocidente Escolar. Mas a observação focada nos locais e nas regiões, representados pelos municípios em Portugal, Brasil, Espanha revela sensibilidades próprias.

O presente relatório de estágio curricular pretende demonstrar a globalidade do trabalho efetuado aquando do estágio no Museu Nacional de Arte Antiga. Procura também evidenciar como o mesmo suscitou interesse em desenvolver uma investigação acerca da Exposição Retrospectiva de Arte Ornamental Portugueza e Hespanhola, de 1882. No concernente à investigação, numa primeira fase, o estudo será relativo à Exposição de Arte Ornamental, bem como aos seus antecedentes. O objetivo é compreender de que modo esta impulsionou a criação, em 1884, do predecessor do Museu Nacional de Arte Antiga, o Museu Nacional de Belas-Artes e Arqueologia. Numa segunda fase, analisar-se-á as primeiras instituições museológicas que contribuíram para a inauguração deste último.

A surdez é uma patologia com grande prevalência mundial, com maior impacto nos países em desenvolvimento. As repercussões que esta tem verificam-se não só ao nível da relação com o ambiente e com os outros, mas também ao nível do próprio indivíduo, principalmente no cérebro. O cérebro apresenta uma capacidade de plasticidade neuronal, sendo maior pouco após o nascimento decrescendo desde então, que, quando privado sensorialmente, permite a adaptação a uma realidade sem estímulos sensoriais auditivos. Assim, observamos que as áreas privadas de inputs são recrutadas por outras áreas sensoriais no processamento de estímulos, nomeadamente visuais e somatossensoriais. A esta capacidade de uma área utilizar outra que se encontrava inutilizada denominamos de plasticidade cross-modal. Uma boa estimulação auditiva durante os primeiros anos de vida é essencial para a maturação do córtex auditivo. Desta forma existem diferenças em relação ao impacto da hipoacusia, especialmente a severa ou profunda, de acordo com a idade em que se instala. Assim, em idade pré-lingual, ou seja, aquela que ocorre antes do início da fala, constata-se uma maior plasticidade cross-modal visual e somatossensorial no córtex auditivo. Da mesma forma, em indivíduos com surdez pós-lingual severa a profunda também se verificou esta plasticidade, ainda que num grau mais ligeiro, pois a plasticidade nesta altura da vida é menor. Verificou-se ainda em adultos que quanto menor a gravidade da hipoacusia menor era a plasticidade cross-modal. Os implantes cocleares permitem restaurar a audição em casos de surdez sensorial. No entanto, observamos uma grande variabilidade relativamente aos outcomes destes, que não pode ser explicada apenas por fatores pré-implantação. Este Trabalho Final de Mestrado pretende rever os achados presentes na bibliografia e averiguar que impacto a plasticidade cross-modal pode ter nos outcomes dos implantes cocleares e se esta pode servir como prognóstico destes.
