Medication time out as a strategy for patient safety: reducing medication errors
ABSTRACT Objectives: to analyze the implementation of the medication time out strategy to reduce medication errors. Methods: this is a quantitative, cross-sectional, inferential study, with direct observation of the implementation of the medication time out strategy, carried out in a cardiac intensive care unit of a university hospital in Rio de Janeiro. Results: 234 prescriptions with 2,799 medications were observed. Of the prescriptions analyzed, 143 (61%) had at least one change with the use of the strategy. In the prescriptions altered, 290 medications had some type of change, and 104 (35.9%) changes were related to potentially harmful medication. During the application of the strategy, prescriptions with polypharmacy had 1.8 times greater chance of presenting an error (p-value = 0.031), which reinforces the importance of the strategy for prescriptions with multiple medications. Conclusions: the implementation of the medication time out strategy contributed to the interception of a high number of medication errors, using few human and material resources.

Consolidation of new public management in nursing education: repercussions to the Unified Health System
ABSTRACT Objectives: to analyze the ways in which neoliberalism has consolidated itself in the public university and in university teaching in nursing; and what interferences it has produced in the pedagogical conceptions and practices of nurse educators. Methods: this is a qualitative research based on Institutional Analysis and conducted in a public university. Results: the data produced with the nursing teachers revealed the consolidation of the New Public Management in the university teaching of the professor-nurse, which is in contradiction with the formative assumptions for the Unified Health System. Final Considerations: it is noticeable how the university and the university teaching in nursing are already impregnated by neoliberal logic. This will possibly have repercussions on the training of professionals for the Unified Health System.

Instrument to evaluate the behavioral intention of hypertensive people when taking anti-hypertensives
ABSTRACT Objectives: to build and analyze the validity of an instrument to assess the behavioral intention of individuals with hypertension to take antihypertensive pills. Methods: methodological study based on the theory of planned behavior, developed in three stages: preparation of the instrument, validation of content and pre-test with the target audience. The content has been validated by six experts. The Content Validity Index was applied. Results: five nurses and one psychologist participated, with experiences in systemic arterial hypertension and/or planned behavior theory. The first version of the instrument contained 40 items. After analysis, the second version originated, with 36 items. 100% agreement was obtained for the scope of the constructions. The pre-test showed clarity and understanding of the items by the target audience. Conclusions: the instrument is a reliable and innovative technological product for use in populations with systemic arterial hypertension and beliefs similar to those identified in the surveyed group.

Strategies of nurse-midwives in relation to working conditions in maternity hospitals
ABSTRACT Objectives: to understand the working conditions and strategies adopted by nurse-midwives in maternity hospitals. Methods: a qualitative, descriptive, exploratory study with 20 nurse-midwives from public maternity hospitals in the city of Rio de Janeiro. Data were collected from June to September 2018 through semi-structured interviews, submitted to thematic content analysis and discussed in the light of the psychodynamics of work. Results: working conditions are inadequate due to poor infrastructure and resource deficit. Therefore, they develop defensive strategies to mitigate suffering, avoid destabilization of professional identity and minimize losses on care, through material purchase, lunch hour abdication, task reorganization and break implementation. Final Considerations: the strategies adopted hide work precariousness and suggest alienation of workers, evidencing the need to foster political awareness of this collective to promote concrete transformations in their work reality.

Costs of healthcare-associated infections in an Intensive Care Unit
ABSTRACT Objectives: to evaluate hospitalization costs of patients with and without Healthcare-Associated Infections an Intensive Care Unit. Methods: a retrospective case-control study. Data collection was retrieved from the medical records of Intensive Care Unit of a medium-sized public hospital in Goiás-Brazil. For each case, two controls were selected. Data on socioeconomic, clinical, and hospital costs were collected. To verify associations between variables, Odds Ratio and linear regression were calculated. Results: a total of 21 patients diagnosed with Healthcare-Associated Infections and 42 controls were evaluated. The hospitalization cost for patients with infection was four times higher than for non-infection patients (p-value<0.001). There was an association between infection and higher mortality (p-value <0.001), longer hospital-stay (p-value =0.021), and higher hospital costs (p-value =0.007). Conclusions: hospitalization costs of diagnosed Healthcare-Associated Infections patients are high compared to those who do not have this diagnosis.

The approach to the environmental issues in postgraduate nursing in Brazil
ABSTRACT Objectives: to analyze the insertion of environmental issues in the postgraduate stricto sensu in nursing in Brazil. Methods: quali-quantitative research, of exploratory approach, with documental research in the Sucupira Platform and in sites of the 112 courses stricto sensu in Nursing in Brazil, in 2018. We analyzed the curricular matrixes; course menus; teaching profile; dissertations and thesis (2004-2016); and bibliographic productions of the four-year evaluation 2013-2016. The analysis of quantitative data was performed by statistical methods; and qualitative data, by the Content Analysis Technique, outlined by Bardin. Results: the results demonstrate incipiency in the treatment of environmental issues. Regarding the theme, we highlight those that refer to socio-historical-cultural aspects in relation to the more direct environmental approach and facilitator of environmental conservation and consequent promotion of health. Final Considerations: The analysis allows identifying the fragility of the environmental approach in the analyzed programs, being necessary to improve the cross-sectionality of environmental issues.

Health personnel, family relationships and codependency of psychoactive substances: a phenomenological approach
ABSTRACT Objectives: to reflect on the daily life of family members dependent on psychoactive substances and the role of health professionals in Primary Care. Methods: a descriptive investigation outlined in Maffesoli’s phenomenological approach, carried out at a Basic Health Unit in Minas Gerais with eight family members dependent on psychoactive substances. Sociodemographic characterization data and in-depth interviews were collected and analyzed with support of NVivo Pro11® software. All ethical and legal aspects were met. Results: physicians were the professionals most mentioned in participants’ statements, who took on a dubious role with other health professionals: welcoming (positive perceptions about care received) and neglect (negative perceptions). Final Considerations: dependence on psychoactive substances imposes a burden on persons, their family members and health professionals. The role of establishing good relationships in the triad professionals-users-family members of primary care is inseparable in favor of treatment and preservation of their mental health.

Development, validation and application of clinical simulation scenarios for assessment of stomatherapy specialists
ABSTRACT Objectives: to build and validate three clinical simulation scenarios and report the application with candidates for the specialist’s degree in stomatherapy. Methods: methodological study, building three scenarios and evaluation checklists; content validation with judges, using content validity index and Modified Kappa Coefficient; pre-test and application. Results: scenarios built based on nursing care for: 1. insufficiency and venous ulcer; 2. demarcation of intestinal stomia; and 3. Clean intermittent catheterization. In the content validation of the 24 items appreciated, 83%, 80%, and 92% were validated without change. In the pre-test, the objectives and checklists were adjusted. In the application, to standardize the evaluation, actors and evaluators were trained previously, and each candidate passed the three stations. Conclusions: scenarios built and with validated content, based on evidence and covering the three areas of stomatherapy. The pre-test allowed for adjustments in the scenarios, and the candidates achieved the expected objectives.

Anayde Corrêa de Carvalho: life and contributions to Bahia’s Nursing School
ABSTRACT Objectives: to analyze the life and career of Anayde Corrêa de Carvalho and her contributions to nursing in Bahia. Methods: oral history of her life and documentary research, with thematic content analysis. Results: three categories were found: Personal Trajectory of Anayde Corrêa de Carvalho; The School of Nursing of Bahia; and School of Nursing of the University of São Paulo and Brazilian Nursing Association. Final Considerations: this study made it possible to analyze the life of an ethical and committed nurse, who dedicated herself to nursing education and research and contributed significantly to the construction of the image and development of nursing as a profession in Bahia.

Feelings, experiences and expectations of kidney transplant individuals and challenges for the nurse
ABSTRACT Objectives: to identify feelings, experiences, and expectations of kidney transplant patients, generated from the diagnosis of chronic renal disease until the post-transplant period, highlighting the challenges for nurses to incorporate individualized care to cope throughout the disease process. Methods: qualitative, descriptive research, carried with seven kidney transplant patients, in the city of Manaus, State of Amazonas. The data analysis followed the methodological referential of Bardin’s content analysis. Results: the diagnosis of the disease was experienced negatively, and hemodialysis was described as an imprisonment and health decline. The transplant meant an improvement in quality of life. The main difficulties were lack of a specialized hospital and low immunity. Conclusions: the nurses’ approach of chronic renal patient and with the renal transplantation favored the discovery of solutions facing the demands of the disease and allowed greater capacity to implement individualized care, surrounding a relationship of trust and respect.

Therapeutic itinerary of people with leprosy: paths, struggles, and challenges in the search for care
ABSTRACT Objectives: to understand how the therapeutic itineraries of people affected by leprosy are processed. Methods: this is a descriptive, qualitative study, conducted in April 2018 in Barão de Grajaú in Maranhão, with interviews in the form of narratives of seven patients who had a late leprosy diagnosis. Results: the search for diagnosis is a major difficulty in accessing health services, resulting in a late diagnosis and, consequently, with the presence of visible deformities. It was noticed that the health units do not have a flow, nor protocols for comprehensive treatment, and these people are referred to a referral unit in another state to perform sputum smear microscopy. Final Considerations: leprosy control actions need reformulations that seek the relationship between operational activities, epidemiological indicators and risk factors, in accordance with the real needs of each region, thus highlighting the gaps evidenced in the therapeutic itineraries.

Organization and planning for the creation of the HIV testing and counselling center
ABSTRACT Objectives: To analyze the process of creating the HIV Testing and Counselling Center in the municipality of Nova Iguaçu, in the state of Rio de Janeiro. Methods: qualitative research, with a historical approach. The direct sources consisted of written documents and 11 oral testimonies. Results: in order to create this Testing and Counselling Center, investment was made in the implementation of a Municipal Management Unit for the STD/AIDS Program; in the elaboration of the Project for the Creation of the Testing and Counselling Center; in the process of attracting, selecting and training human resources; and choosing the right place for its operation. Final Considerations: the nurse who coordinated this investment had the support of the authorities of that municipality and had an authorized speech on HIV/AIDS in the municipality of Nova Iguaçu; planned each stage of this service and used effective strategies to create the Testing and Counselling Center.

The use of selfbody in nurses work in hospital care: an ergology approach
ABSTRACT Objective: to know how happens the nurse use of selfbody in the emergency room. Methods: qualitative study, performed with 23 nurses in an adult emergency room at a university hospital in the South of Brazil. The data gathering was through documental research, systematic observation, and semi-structured interview. According to the thematic modality, the data analysis was carried out anchored in the theoretical reference of ergology. Results: it was evidenced the use of selfbody in the work of the nurse, as much in the development of managerial activities as assistance, especially: in the organization of the environment, in the performance in intercurrences and definition of priorities of attendance, as well as in the conduction of the activities of each work shift, together with the nursing and multi-professional team. Final considerations: the nurse makes use of selfbody at work in the emergency room, based on values, knowledge, and experience, considering the organization of the work process and better nursing assistance.

Nurse contribution to the construction of health policy in the state of Bahia (1925-1930)
ABSTRACT Objective: to analyze the contribution of nurses to the construction of health policy in the state of Bahia, from 1925 to 1930. Methods: qualitative research, from a historical nature. Data were retrieved from five public archives, organized in a documentary corpus, and analyzed based on the health political analysis and the social control concepts, health policy and public health. Results: from 1925 to 1930, the State seized the work of the woman/nurse and established it in public health. This fact enabled the nurse’s contribution to the construction of the health policy of the state of Bahia, which took place by the implementation of sanitary education actions, home visits and hygienic surveillance. Final considerations: the female nurse’s work made the health policy of the state of Bahia viable and was an ideal instrument to access homes and instruct/advise people in their daily lives to adopt behaviors that prevent the occurrence and, above all, the spread of diseases.

Nurses and the Manchester: rearranging the work process and emergency care?
ABSTRACT Objective: to understand the changing roles of nurses in labor division organization in hospitals from the Manchester Triage System implementation in an emergency hospital. Methods: this is an ethnographic study that used different production techniques and data analysis. Results: the Manchester Triage System organized flows and places resulting in quality of care and changes in work processes. Conflict relationships related to disagreements in risk stratification were present. Final considerations: the traditional roles of nurses have been transformed, but it cannot be said that there was a structural change in their position in labor division organization in hospitals. The frontiers of autonomy, therefore of increasing the professionalization of nurses, are neither fixed nor stable, expanding or contracting according to the micropolitical changes in the governance of care.

Social representation of reproductive biotechnologies among nurses working in sexual and reproductive health
ABSTRACT Objectives: to compare the social representations of reproductive biotechnologies among sexual and reproductive health nurses, and their links with professional practice. Methods: an analytical, comparative, qualitative research, supported by the Theory of Social Representation, carried out in May/2014-February/2015, with 32 nurses from the city of Rio de Janeiro. Semi-structured interview, analyzed by ALCESTE® software. Results: nurses not active in assisted human reproduction represent reproductive biotechnologies as unnatural methods of human reproduction, assessing the practice in this field as generalist and bureaucratic. Those who work represent as auxiliary and supporting nature for heterosexual couples, considering the innovative and specialized practice. Final considerations: each group presented specific contents and dimensions about reproductive biotechnologies. The representations are centered on moral, normative and ideological personal values, anchored in the traditional conceptions of human and family reproduction, but also collective, acquired in the professional routine, showing group identity and its distinct practices considering reproductive biotechnologies.

Quality of life and changes in the social dimension of hematopoietic stem cell transplants recipients
ABSTRACT Objectives: to assess/correlate health-related quality of life with the social dimension of hematopoietic, autologous and allogeneic stem cell transplant patients in the three years post-transplant. Methods: longitudinal, observational study with 55 patients, in a reference hospital in Latin America, from September 2013 to February 2019, using the Quality of Life Questionnaire-Core and Functional Assessment Cancer Therapy Bone Marrow Transplantation. Results: A total of 71% underwent allogeneic transplantation. The social dimension had low averages since the baseline stage (55, 21) and low scores (56) for quality of life in pancytopenia. There was a significant positive correlation between social dimension, quality of life in pancytopenia (p<0.01) and follow-up after hospital discharge (p<0.00). There is a significant difference (p<0.00) throughout the stages, however, not in terms of the type of transplant (p>0.36/0.86). Conclusions: patients with better assessments in the social dimension have a better quality of life. Interventions focusing on the multidimensionality of the quality of life construct are necessary.

Activity limitations in leprosy and their association to cognition and neuropsychiatric symptoms
ABSTRACT Objectives: to analyze activity limitations in patients with leprosy and their association to cognition and neuropsychiatric symptoms. Methods: this cross-sectional design study included sixty patients with multibacillary leprosy assisted at a reference center. Participants were divided according to their physical disability and submitted to instruments assessing daily activities (basic and instrumental), cognitive functions, and neuropsychiatric symptoms. The order of the instruments was random to avoid any interference of test sequence on the results. Results: fifty-five percent of the participants presented physical disabilities. All patients were independent in performing basic activities but sixty-six percent presented difficulties in performing instrumental activities. Cognitive decline and neuropsychiatric symptoms were seen in advanced stages of the disease, but they were more associated to patient age than to leprosy. Conclusions: leprosy patients presented physical disabilities and difficulties in performing instrumental daily activities. Cognitive decline and neuropsychiatric symptoms were more associated to aging than to leprosy itself.

Route of chronic kidney patients foreigners in the search for health care in a border area
ABSTRACT Objectives: to describe the journey of foreigners with chronic kidney diseases searching for treatment in the Brazilian Unified Health System in a border area. Methods: this is a qualitative research conducted with thirteen foreign participants. Data were collected by interviews and analyzed based on the theoretical-methodological framework of the therapeutic itinerary. Results: empirical evidence showed seven thematic categories, indicating poor illness perception; poor family support throughout treatment; social aspects such as work, income, and housing as sources of distress; search for alternative treatments; interrupted search for healthcare abroad due to financial problems; journey to healthcare-seeking in Brazil eased by family members and social networks. Final considerations: the asymmetries within the Paraguayan and Brazilian health systems spur the search for cross-border integration actions. Identifying another country’s assistance aspects may help in planning more welcoming policies in border regions.

Perceptions of senior nursing students regarding clinical decision-making
ABSTRACT Objectives: to understand the perceptions of senior students in an undergraduate nursing course at a nursing college in Lisbon regarding skills related to clinical decision-making learned during their initial training. Methods: qualitative study, of an exploratory and descriptive nature. Data collection occurred by carrying out a focus group with eight senior students in an undergraduate nursing course. Data treatment applied content analysis, performed with twebQDA® software. Results: data were analyzed according to the four dimensions of Tanner’s model of clinical judgment, and noticing and interpreting stood out as the most influential in clinical decision-making. Theoretical and clinical knowledge, validation of care planning, prioritization, and capacity to discuss and debate about situations emerged as the most representative skills. Final considerations: students make decisions by means of a complex process by using the knowledge and skills learned during their training.
