Nursing APHMÓVEL: mobile application to register the nursing process in prehospital emergency care

ABSTRACT Objectives: to develop an application for a mobile device for the registration of the Nursing Process by nurses of the Mobile Emergency Care Service. Methods: applied research with technology development based on software engineering and Apple's Human Interface Guidelines. It had the support of an application developer and a designer. The proposal was built in four steps (scope definition, planning, prototype creation and development). Results: the application "Nursing APHMóvel" allows to record the steps of the Nursing Process, such as history, nursing diagnoses, outcomes and interventions with the possibility of storing the data and/or file in printable format. The technological innovation has location features, scales with automatic sum of items, touchscreen, and offline operation. Conclusions: it was possible to develop the application with the potential for computerized documentation of the Nursing Process by nurses working in the Mobile Emergency Care Service.

Innovative actions developed by nurses in primary health care

ABSTRACT Objectives: to understand the work process dimensions related to innovative actions developed by nurses in Primary Health Care. Methods: qualitative, descriptive study, developed in Primary Health Care in a city in the Southern Region of Brazil. Seventy-six nurses, who worked in management and assistance, participated in this study through semi-structured interviews. After data processing by IRAMUTEQ software, the textual analysis occurred by descending hierarchical classification. Results: forty-two innovative actions, considered by the participants as new ways of working, were identified. There was a predominance of innovative actions related to the Management and Assist dimensions; we noticed the fragility of the actions to contemplate all the nurse’s work process dimensions. Final Considerations: the predominance of innovative actions was related to users’ assistance and better conditions in the teamwork process, besides highlighting the role of nurses in the perspective of a new way of working in health services.

Religious/spiritual experiences, quality of life and satisfaction with life of hospitalized octogenarians

ABSTRACT Objectives: to assess quality of life (QoL) and satisfaction with life (SWL) indices and verify whether the frequency of religious and spiritual experiences is associated with QoL and SWL in hospitalized octogenarians. Method: this is a cross-sectional study, with 128 octogenarians. World Health Organization QoL instruments and Scales applied: Daily Spiritual Experience (DSES) and Satisfaction With Life (SWLS). Results: more committed domains related to QoL and SWL: autonomy and physical capacity. The higher the score in DSES, the higher the scores in the psychological domains and past, present, and future QoL activities. The higher the score in DSES, the higher the score in the social involvement aspect. Conclusion: the results of this study showed that the higher frequency of religious and spiritual experiences of hospitalized elderly people was associated with better QoL and SWL. It is emphasized that religious and spiritual experiences should be explored in the hospital therapeutic context.

Intramuscular injection safety without aspiration in the ventro-gluteal region during vaccination: randomized clinical trial

ABSTRACT Objectives: to compare adverse events after administrating hepatitis A vaccine intramuscularly in the ventro-gluteal region between techniques with and without aspiration. Methods: randomized double-blind clinical trial, using hepatitis A vaccine (inactivated) in the ventro-gluteal region, with a sample of 74 participants in the intervention group, vaccinated with the slow injection technique without aspiration, and 74 participants in the control group undergoing slow injection with aspiration. Daily assessment of participants was carried out in the 72 hours after vaccination, in order to ascertain local, systemic adverse events, local and contralateral temperatures. Results: the occurrence of local and systemic adverse events was homogeneous between the groups in the three days after vaccination (p>0.05). There was no influence of sex, race, pre-existing disease and use of medication. Conclusions: the intramuscular vaccination technique without aspiration in the ventro-gluteal region is safe for adverse events following immunization compared to the conventional technique with aspiration.

Quality of life in people with coronary artery disease: translation and cross-cultural adaptation of a questionnaire

ABSTRACT Objective: to carry out translation, cross-cultural adaptation to Portuguese and assess the reliability of the four versions of the Coronary Revascularization Outcome Questionnaire - adapted. Methods: a methodological study of translation and cross-cultural adaptation into Portuguese, as proposed by Beaton et al., and reliability analysis, by calculating Cronbach's alpha coefficient and item-total correlation of the four versions of the Coronary Revascularization Outcome Questionnaire - adapted. Results: the adaptations made facilitated the understanding of the items. All domains had Cronbach's alpha above 0.70, except two in the post-percutaneous coronary intervention version and two in the post-myocardial surgical revascularization version. All items had item-total correlation values greater than 0.20, except two in the post-percutaneous coronary intervention version, one in the pre-coronary intervention version and six in the post-myocardial surgical revascularization version. Conclusion: the cross-cultural adaptation to Portuguese was considered satisfactory. Pre-procedure versions were considered reliable, while the post-procedures require other psychometric analyses.

Lessons from the past in the present: news from the Spanish flu pandemic to COVID-19

ABSTRACT Objective: to analyze the health conducts for combating the Spanish flu and its relationship with the COVID-19 pandemic in Rio de Janeiro. Methods: study from the perspective of microhistory, with analysis of articles published in Revista da Semana on the Spanish flu, having as criteria publications with the term "epidemic" referring to the Spanish flu or influenza in the period of the epidemic outbreak in Rio de Janeiro, Federal District. Results: 18 records, organized in three thematic axes: political, 4; social, 11; care, 3, were found. Final considerations: the lessons left by the Spanish flu epidemic in coronavirus times will be changes in the political, social and care field as marks of major epidemics.

(Lack of) knowledge of mothers about sickle cell trait and disease: a qualitative study

ABSTRACT Objective: to analyze the understanding of mothers about sickle cell disease and/or trait of the family from a diagnosed child. Methods: this is a qualitative study, using a semi-structured interview with 23 mothers, at a sickle cell disease outpatient clinic of a public institution, from October to December 2017. Analysis was thematic. Results: all participants had sickle cell trait as well as the parents of their children. Twenty children were diagnosed with sickle cell disease by Heel Prick Test, and three, after hospitalization due to the disease. Most did not know how to report the presence of the trait or disease in relatives other than nuclear. Final considerations: diagnosis cannot be restricted to the result of neonatal screening, requiring that preventive information on sickle cell crises be reinforced. It is recommended to search for other affected relatives to learn about their genetic condition, reflecting on their reproductive decisions.

Evaluation of the health level of the elderly: patient care team considerations

ABSTRACT Objective: To understand the differences of physicians, nurses, and social workers in the evaluation of the health status of the elderly. Methods: A cross-sectional quantitative study, using descriptive statistics. Non-probabilistic sample, consisting of 291 participants from three professional categories: 71 (24.4%) physicians, 192 (66%) nurses, and 28 (9.6%) social workers. We used a questionnaire including the variables: sociodemographic characteristics and instruments used for evaluation. Results: Instruments with greater utility for the evaluation of the elderly: for physicians, Mini Mental State Examination; for nurses, Braden scale; and for social workers, genogram. In the physical examination, the data most collected by physicians and nurses are the vital signs; and by social workers, the condition for performing the Activities of Daily Living. Conclusions: The evaluation of the elderly is based on a diversity of instruments and is an area in which health and social professionals need to share information.

Evaluating the psychometric properties of the eHealth Literacy Scale in Brazilian adults

ABSTRACT Objective: To translate and adapt the eHealth Literacy Scale for the cultural reality of Brazil and to evaluate the psychometric properties of its Brazilian Portuguese version. Methods: The instrument was translated and adapted to Brazilian Portuguese and applied to a sample of 502 individuals from 18 to 80 years old who lived in the surrounding areas of six Family Health Units of a city in the countryside of the state of São Paulo, Brazil. The data was evaluated using exploratory and confirmatory factor analysis, item response theory, and instrument reliability measures (Cronbach’s alpha and McDonald’s omega). Results: The eHealth Literacy Scale - Brazilian version (eHEALS-Br) presented an excellent internal consistency (α = 0.95 e ω = 0.95), with only one dimension and an explained variation of 81.79%. Conclusions: The Brazilian version of the instrument showed excellent psychometric properties to measure the levels of digital health literacy in adults from the country.

Career commitment and career entrenchment among Primary Health Care workers

ABSTRACT Objective: Assess levels of career commitment and career entrenchment among Primary Health Care workers. Methods: This Cross-sectional study addressed 393 workers using the Brazilian versions of the Career Commitment Measure (CCM) and Career Entrenchment Measure (CEM). Results: Levels of Career commitment [75.5-77.5] were higher (p<0.001) than Career Entrenchment [66.7-69.2]. Identity levels [82.7-85.5] were higher (p<0.001) than Investment levels [60.4-65.0]. Career resilience levels [75.1-79.2] were higher (p<0.001) than Emotional costs [69.0-72.1]. Planning levels [64.2-67.1] were lower (p<0.001) than levels of limitedness of career alternatives [68.1-71.0]. Conclusion: The highest scores were obtained in Career commitment, showing the workers’ identification and positive relationship with their careers, that is, these workers remain in Primary Health Care services because they identify themselves with their professions.

Self-care of elderly people with diabetes mellitus and the nurse-patient interpersonal relationship

ABSTRACT Objective: To analyze the self-care activities of elderly people with diabetes mellitus and its correlation with the nurse-patient interpersonal relationship. Methods: Cross-sectional quantitative study, with 144 elderly people followed up in Family Health Units, with data collected by the Diabetes Self-Care Activities Questionnaire and Interpersonal Relationship Questionnaire in Nursing Care, being analyzed by descriptive and inferential statistics. Results: There was a greater accomplishment of the activities Take insulin injections as recommended (6.74), Take diabetes medications as recommended (6.55) and Take the indicated number of diabetes pills (6.52). The interpersonal relationship showed moderate effectiveness (80.6%). The correlation between self-care with diabetes and interpersonal relationships showed a positive and significant value in the dimension Specific feeding. Conclusion: The effectiveness of the interpersonal relationship in nursing care resulted in greater compliance with activities related to specific food.

Factors associated with presenteeism in nursing workers

ABSTRACT Objective: to analyze factors associated with presenteeism in nursing workers with sociodemographic variables, health and work conditions, productivity and musculoskeletal symptoms. Methods: this is a cross-sectional, descriptive and analytical study, with 306 nursing workers from a hospital and municipal emergency room in a Brazilian capital. The Stanford Presenteeism Scale, the Work Limitations Questionnaire, the Nordic Musculoskeletal Questionnaire and a demographic questionnaire on nursing professionals’ working conditions and health were used. Bivariate and multivariate analyzes were performed, respecting a significance level of 5%. Results: presenteeism was found in 43.8% of professionals and significant associations with CLT work (p=0.002), workplace - Intensive Care Units (p=0.008), physical exercise twice a week (p=0.008), presence of musculoskeletal symptoms, with low back pain being representative (p=0.001). The productivity loss was 8.8. Conclusions: the study confirms a high rate of presenteeism among nursing workers.

Judicialization of nursing malpractice in perioperative care, and delivery and birth assistance

ABSTRACT Objectives: to analyze the legal outcomes of malpractices in perioperative care, and delivery and birth assistance related to nursing, from the perspective of legal support for malpractice prevention. Methods: an exploratory, documentary, qualitative study, based on the cases tried by the Court of Justice of the State of Paraná, available online until April 2018. For the data analysis, we codified the processes and summarized the judicial outcome by the severity of the malpractice. Then, we recommended practices for the prevention of each case we presented. Results: among the thirteen processes analyzed, eight corresponded to the perioperative period (mainly electrocautery burn), and five to nursing care for delivery and birth. The severity of the cases was high (n=7). The judicial outcome of most cases (n=11) was the conviction of the institution. Conclusions: despite the multifactorial nature of the malpractices, the identified ones are preventable since there is a description of good practices.

Healthy aging promotion model referenced in Nola Pender’s theory

ABSTRACT Objectives: to model the process of promoting healthy aging from the conceptual analysis proposed by Walker and Avant and Nola Pender’s Health Promotion Model. Methods: this is a descriptive and theoretical study, with a qualitative approach. Elements resulting from conceptual analysis were used to model a healthy aging promotion process based on Nola Pender’s Health Promotion Model. Results: in conceptual analysis, seven antecedents, seven attributes and three consequences of the concept of healthy aging were identified. Final Considerations: the proposed Health Promotion Model represents a structure that in an instrumental way can guide the nursing process application in gerontological clinical practice. This can guide nurses in identifying diagnoses, establishing outcomes and implementing interventions aimed at promoting the elderly’s health.

Care for the spiritual dimension provided by caregivers in a nursing home

ABSTRACT Objectives: to understand care for the spiritual dimension provided by caregivers in a Nursing Home. Methods: this is a qualitative research, carried out in a geriatric center of a philanthropic hospital in the city of Salvador, Bahia. Eighteen formal caregivers participated, through a semi-structured interview, between January and February 2019. The data were analyzed in the light of Jean Watson’s Theory of Transpersonal Caring. Results: formal caregivers discuss the spiritual dimension based on older adults’ religious beliefs, encourage religious practices and exercise spiritual care according to older adults’ physical, emotional and spiritual demands. Final Considerations: formal caregivers understand that older adults’ religious or spiritual experiences should be included in their work routine. Care for the spiritual dimension occurs by stimulating faith in God, encouraging religious practices and embracing their beliefs in the face of physical, emotional and spiritual demands.

Characterization of obstetric care developed in teaching hospitals in a capital of northeast Brazil

ABSTRACT Objective: to describe the obstetric care developed in teaching hospitals (TH) in the city of Maceió-AL, intended for high-risk pregnancies. Methods: Retrospective cohort study performed between June and November 2018 with 291 women who received assistance with vaginal delivery, cesarean section, or abortion process in teaching hospitals selected as settings for this research. Data collection allowed the characterization of the interviewees as well as the assistance received and its relationship with obstetric violence. The analysis was performed using descriptive and analytical statistics with the aid of the Epi Info software (version 7.2.0.1) and measures that allowed the comparison of means and proportions. Results: All women reported at least one violent situation, contrary to recommendations based on scientific evidence. Conclusion: It becomes necessary to optimize professional training to deconstruct obstetric care based on medicalization and pathologization of pregnancy and strengthen science-based care.

Prevalence and factors associated with sepsis and septic shock in oncological patients in intensive therapy

ABSTRACT Objectives: to analyze factors associated with sepsis and septic shock in cancer patients in the Intensive Care Unit. Methods: cross-sectional, retrospective study with a quantitative approach, with a sample of 239 patients in an oncology hospital. Secondary data from medical records were used. The outcome variable was “presence of sepsis and/or septic shock”; and exposures: sex, length of stay, origin, use of invasive procedures and primary tumor site. Descriptive, bivariate analyzes and multiple logistic regression models were performed. Results: the prevalence of sepsis was 95% CI: 14.7-24.7 and septic shock of 95% CI: 37.7-50.3. In the multiple analysis, sepsis and/or septic shock were associated with hospital stay longer than seven days, being from the Emergency Department, presence of invasive procedures and hematological site. Conclusions: sepsis and/or septic shock in cancer patients were associated with clinical characteristics and health care factors.

Patient safety in nursing technician training

ABSTRACT Objectives: to analyze the contents on patient safety in the training of nursing technicians. Methods: a documentary study, conducted in three technical nursing courses at a public university in northeastern Brazil, based on the Multi-Professional Patient Safety Curriculum Guide, published by the World Health Organization. Results: we found that, of the 26 subjects in each course, the tracking terms were found in 22 subjects in the A/C courses, 23 in the B course. The topics of the guide with the highest number of terms were the improvement in medication safety, with 85 terms (22.6%), and Infection prevention and control, with 75 terms (20%). The contents do not express the comprehensiveness of patient safety education; some subjects had this focus, while others did not. Conclusions: the documents revealed gaps in the contents related to patient safety and demonstrated that they are addressed only in the course syllabus and discipline plans.

Risk classification in a pediatric service: evaluation of the structure, process, and outcome

ABSTRACT Objectives: to evaluate a pediatric risk evaluation and classification service in the aspects of structure, process, and outcome. Methods: a cross-sectional, descriptive, and exploratory study in which qualitative data complement the results. Data collection by non-participant observation, interview, and consultation of medical records. Results: we observed two hundred and four visits. The service works in a makeshift room and without privacy. The nurses did not use various materials/equipment available for evaluation, rarely performed a physical examination, used the interview most of the time, and did not consult the protocol. Parents/companions received no information, and medical records showed incomplete records. Nurses classified 69.1% of the children as non-emergency. The most frequent outcome of the visits was discharge. Conclusions: the service showed weaknesses in the physical structure, in the risk classification process, and the outcomes, but it presents a good resolution in the assistance. We recommend a revision in the protocol used and sensitize nurses about the work process and communication with parents/companions.

Nurses’ performance in palliative care: spiritual care in the light of Theory of Human Caring

ABSTRACT Objectives: to analyze nurses’ role in assisting patients in palliative care, with emphasis on the spiritual dimension, in the light of Theory of Human Caring. Methods: this is an exploratory, qualitative study, carried out in a hospital in João Pessoa, Paraíba, between August and December 2019, with 10 nurses. For data collection, semi-structured interviews were used. For analysis, we opted for content analysis. Results: the spiritual dimension of care is contemplated by several religious and spiritual practices. These are respected and encouraged by nurses, although there is difficulty in providing care for the spiritual dimension. Final Considerations: nurses have attitudes consistent with Jean Watson’s Theory and apply the Caritas Process elements during assistance to patients’ spiritual dimension in palliative care.
